目的:比较动画视频与告知-演示-操作(Tell-Show-Do,TSD)技术在儿童乳磨牙预成冠修复治疗中缓解疼痛及焦虑水平的临床效果.方法:选择来我院口腔科门诊就诊的 4~6 岁因第一乳磨牙患邻面龋需行金属预成冠修复的 90 例患儿作为研究对象,随机分为 2 组,每组各 45 颗患牙.对照组仅在金属预成冠修复的治疗过程中采用告知-演示-操作(TSD)技术进行行为引导,引导患儿完成治疗;实验组在TSD方法的基础上,在治疗的过程中播放患儿喜爱的动画视频分散患儿的注意力,以引导患儿顺利完成治疗.通过Frankl临床行为分级评定量表评估患儿治疗前后焦虑等级,Venham临床焦虑及合作行为评定量表评估治疗过程中的临床合作情况,Wong-Baker面部表情量表评估局部镇痛时疼痛感知程度,比较两种干预措施在缓解患儿疼痛及焦虑行为的效果.结果:所有患儿均完成金属预成冠修复的治疗.两组治疗前后Frankl量表、治疗中Venham量表评级以及疼痛等级程度差异均有统计学意义(P<0.05),实验组患儿的临床配合度明显提高.结论:动画视频能有效降低患儿焦虑水平及治疗时疼痛程度,提高患儿就诊的配合度与就诊效率.

Objective:To compare the clinical effects of animated video and the Tell-Show-Do(TSD)technique in alleviating pain and anxiety levels in the preformed crown restoration treatment of deciduous molars in children.Methods:A total of 90 children aged 4 to 6 years old who visited the department of stomatology in our hospital and needed metal pre-formed crown restoration due to interproximal caries of the first deciduous molars were selected as the research subjects,and randomly divided into 2 groups,with 45 affected teeth in each group.In the control group,only the Tell-Show-Do(TSD)tech-nique was used for behavioral guidance during the treatment process of metal preformed crown to guide the children to com-plete the treatment,the experimental group,based on the TSD method,played cartoons that the children loved during the treatment process to distract their attention and guide them to successfully complete the treatment.The anxiety levels of the children before and after treatment were evaluated by the Frankl clinical behavior rating scale,the clinical cooperation during the treatment process was evaluated by the Venham clinical anxiety and cooperative behavior rating quantitative scale.And the degree of pain perception during local analgesia was evaluated by the Wong-Baker facial expression scale,and the effects of the two intervention measures in alleviating the pain and anxious behaviors of the children were compared.Results:All the children patients completed the treatment of metal preformed crown repair.The differences between the two groups in the Frankl scale ratings before and after treatment,the Venham scale ratings during treatment,and the degree of pain level were all statistically significant(P<0.05),and the clinical cooperation of the children in the experimental group was significantly improved.Conclusion:Animated videos can effectively reduce the anxiety level of children and the degree of pain during treatment,improve their cooperation during medical visits,and enhance the efficiency of medical treatment.
