目的:分析条带技术联合异种胶原基质用于连续缺失磨牙种植体周角化牙龈增宽的临床效果.方法:选取我院 2022 年 8 月~2024 年 11 月收治的磨牙连续缺失种植修复治疗的患者 122 例作为研究对象,患者随机分为A组(62 例)、B组(60 例),A组患者采用条带技术联合异种胶原基质治疗,B组患者采用游离龈移植进行治疗.观察测量两组患者治疗后各时点种植体周角化龈宽度、软组织厚度;疼痛视觉模拟评分法(visual analogue scale,VAS)对患者治疗后 3,7,14 d点疼痛情况进行评估;并评价修复后种植体周软组织健康状况.结果:A、B组术后即刻、术后2 个月、6 个月角化龈宽度明显大于术前,术后 6 个月明显低于术后即刻,术后A组角化龈宽度明显大于B组(P<0.05),各时点软组织厚度,仅在时点因素中具有统计学差异(P<0.05).术后 6 个月A组牙龈区域组织收缩率明显低于B组(P<0.05),种植体周软组织健康状况均无差异.A、B组术后7、14 d VAS评分明显低于术后3 d,术后14 d明显低于术后 7 d;术后 3 d、7 d时A组VAS评分明显低于B组(P<0.05).结论:条带联合异种胶原基质用于增宽连续缺失磨牙种植体周角化牙龈可获得良好的角化牙龈增宽,能保持种植体周软组织健康,显著减轻患者术后疼痛.

Objective:Analyze the clinical effect of combining strip technology with heterologous collagen matrix for widening keratinized gingiva around continuous missing molar implants.Methods:Totally 122 patients with continuous molar loss treated with implant restoration in our hospital from August 2022 to November 2024 were selected as the research sub-jects,randomly divided into Group A(62 cases)and Group B(60 cases).Group A patients were treated with strip technology combined with heterologous collagen matrix,while Group B treated with free gingival transplantation.The width of keratinized gingiva and the soft tissue thickness around the implant at each time point after treatment in two groups of patients were ob-served,visual analogue scale(VAS)was used to evaluate the pain levels of patients at 3,7,and 14 days after treatment,and the health status of the soft tissue around the implant after repair were evaluated.Results:The width of keratinized gingiva in both Group A and Group B was significantly wider immediately after surgery,as well as 2 and 6 months after surgery,com-pared to before surgery.However,it was significantly narrower at 6 months after surgery compared to immediately after surger-y.Additionally,the width of keratinized gingiva in Group A was significantly wider than that in Group B at 6 months after sur-gery(P<0.05),and the soft tissue thickness at each time point was statistically different only in the time point factor(P<0.05).At 6 months after surgery,the gingival tissue shrinkage rate in group A was significantly lower than that in group B(P<0.05).There was no difference in the health status of the soft tissue around the implant at 6 months after surgery.On postoperative days 7 and 14,VAS scores in group A and B were significantly lower than those on postoperative day 3,and on postoperative day 14,were significantly lower than those on postoperative day 7.On postoperative day 3,day 7,VAS scores in group A were significantly lower than those in group B(P<0.05).Conclusion:The combination of strip and heterogeneous collagen matrix for widening the keratinized gingiva around continuous missing molars can achieve good keratinized gingiva widening,maintain the health of soft tissue around the implant,and significantly reduce postoperative pain in patients.
