Research progress on the mechanism of Euonymus alatus and its active components in the treatment of diabetic kidney disease

糖尿病肾病(diabetic kidney disease,DKD)是糖尿病最为严重且普遍的微血管并发症之一,其发病率呈逐年上升趋势,严重威胁人类健康.鬼箭羽作为一种传统的中药,具有活血通络、破血消癥、解毒杀虫之功效,其与DKD肾络瘀阻的病机高度契合.现代药理学研究表明鬼箭羽中富含黄酮类、三萜类、生物碱类等多种生物活性成分,其治疗DKD的作用机制呈现多靶点、多通路调控特征:通过激活过氧化物酶体增殖物激活受体γ(peroxisome proliferator-activated receptor-γ,PPAR-γ)通路改善胰岛素抵抗;经调节脂肪细胞因子肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)和脂联素(adiponectin,ADPN)维持脂代谢稳态;通过激活核因子E2相关因子2(nuclear factor erythroid 2-related factor 2,Nrf2)/抗氧化反应元件(antioxidant response element,ARE)通路清除活性氧(reactive oxygen species,ROS),以抗氧化应激,并抑制核转录因子(nuclear factor-κB,NF-κB)通路降低TNF-α、白细胞介素-6(interleukin-6,IL-6)等促炎因子释放;同时下调转化生长因子-β1(transforming growth factor-β1,TGF-β1)/同源基因Sma和Mad蛋白(Sma-and Mad-related proteins,Smads)信号轴抑制细胞外基质沉积,延缓肾脏纤维化.本文系统梳理国内外相关文献中鬼箭羽治疗DKD的相关活性成分,结合现代药理学研究成果,深入阐述其作用机制,旨在为鬼箭羽的进一步开发及其在DKD治疗中的深入研究提供科学依据.

Diabetic kidney disease(DKD)is one of the most severe and prevalent microvascular complications of diabetes mellitus.Its incidence is increasing annually,posing a serious threat to human health.Euonymus alatus,a traditional Chinese me-dicinal herb known for its effects in promoting blood circulation and dredging collaterals,breaking blood stasis and resolving mass-es,as well as detoxifying and killing parasites,is highly consistent with the pathogenesis of kidney collateral stasis in DKD.Mod-ern pharmacological studies indicate that Euonymus alatus is rich in various bioactive components,including flavonoids,triterpe-noids,and alkaloids.Its therapeutic mechanism against DKD exhibits multi-target and multi-pathway regulatory characteristics:it improves insulin resistance by activating the peroxisome proliferator-activated receptor-γ(PPAR-γ)pathway;maintains lipid met-abolic homeostasis by regulating adipocytokines tumor necrosis factor-α(TNF-α)and adiponectin(ADPN);exerts antioxidant ef-fects by clearing reactive oxygen species(ROS)through activation of the nuclear factor erythroid 2-related factor 2(Nrf2)/antiox-idant response element(ARE)pathway,while reducing the release of pro-inflammatory factors such as TNF-α and interleukin-6(IL-6)via inhibition of the nuclear factor-κB(NF-κB)pathway;concurrently,it downregulates the transforming growth factor-β1(TGF-β1)/Sma-and Mad-related proteins(Smads)signaling axis to inhibit extracellular matrix deposition and delay renal fibro-sis.This article systematically reviews the relevant active components of Euonymus alatus in DKD treatment reported in domestic and international literature,and elucidates its mechanisms of action in-depth by integrating modern pharmacological research find-ings,for the purpose of providing a scientific foundation for the further development of Euonymus alatus and its in-depth research in DKD therapy.
