基于Th17/Treg免疫平衡探讨艾灸对缺血性脑卒中大鼠脑肠共治机制研究OA

目的:观察艾灸干预对缺血性脑卒中大鼠Th17/Treg细胞的影响,从免疫平衡角度探讨其通过"脑肠共治"改善神经功能的作用机制.方法:120只雄性SD大鼠随机分为空白组(25只)、造模组(95只).75只大脑中动脉闭塞(middle cerebral artery occlusion,MCAO)造模成功大鼠随机分为模型组、益生菌组和艾灸组,每组 25只.益生菌组给予金双歧灌胃,艾灸组给予悬灸"百会""大椎"及双侧"天枢""足三里"穴,每次 20 min,两组每日干预1次,连续7 d.干预结束后,采用Longa评分、TTC染色、H&E染色、ELISA、实时荧光定量PCR、Western blot、免疫组织化学及流式细胞术检测艾灸对MCAO大鼠的影响.结果:与空白组相比,模型组Longa评分升高(P<0.01),脑梗死体积增大(P<0.01),缺血侧脑、结肠组织形态破坏且炎性细胞浸润明显,白细胞介素-10(interleukin-10,IL-10)含量减少(P<0.01),IL-10和叉头框蛋白P3(forkhead box P3,FOXP3)mRNA、蛋白表达水平及阳性表达降低(P<0.01),白细胞介素-17A(interleukin-17A,IL-17A)含量增加(P<0.01),IL-17A和维甲酸相关孤核受体γt(retinoid-related orphan nuclear receptor γt,RORγt)mRNA、蛋白表达水平及阳性表达升高(P<0.01),CD4+、Treg百分比降低(P<0.05),CD8+、Th17 百分比升高(P<0.05).与模型组相比,益生菌组和艾灸组Longa评分降低(P<0.01),脑梗死体积缩小(P<0.01),缺血侧脑、结肠组织形态相对完好,炎性细胞浸润减轻,IL-10含量增加(P<0.01),IL-10和FOXP3 mRNA、蛋白表达水平及阳性表达升高(P<0.01),IL-17A含量减少(P<0.01),IL-17A和RORγt mRNA、蛋白表达水平及阳性表达降低(P<0.01),CD4+、Treg百分比升高(P<0.05),CD8+、Th17百分比降低(P<0.05).结论:艾灸通过脑肠共治改善缺血性脑卒中后神经功能损伤,可能与调节Th17/Treg免疫平衡,抑制脑肠炎性反应有关.

Objective:To observe the effects of moxibustion on Th17/Treg cells in rats with ischemic stroke and to explore its mechanism of improving neurological function through the"brain-gut co-medication"from the perspective of immune homeostasis.Methods:A total of 120 male SD rats were randomly divided into the blank group(25 rats)and the modeling group(95 rats).Among which 75 successful middle cerebral artery occlusion(MCAO)modeled rats were randomly divided into the model group,the probiotic group,and the moxibustion group,with 25 rats in each group.In the probiotic group,gold bifidus was administered by gavage,and in the moxibustion group,moxibustion was administered at the"Baihui"and"Dazhui"points,as well as the bilater-al"Tianshu"and"Zusanli"points,for 20 min each time.Both groups were intervened once a day for 7 consecutive days.At the end of the interventions,the effects of moxibustion on the MCAO rats were detected by the Longa score,TTC staining,H&E stain-ing,ELISA,real-time fluorescence quantitative PCR,Western blot,immunohistochemistry,and flow cytometry.Results:Com-pared to the blank group,the model group exhibited higher Longa scores(P<0.01)and larger cerebral infarct volumes(P<0.01).There was pronounced morphological disruption and inflammatory cell infiltration in the ischemic brain and colon tissues.The level of interleukin-10(IL-10)was decreased(P<0.01),IL-10 and forkhead box P3(FOXP3)mRNA,protein,and posi-tive expressions were reduced(P<0.01).The level of interleukin-17A(IL-17A)was increased(P<0.01),IL-17A and retinoid-related orphan nuclear receptor γt(RORγt)mRNA,protein,and positive expressions were increased(P<0.01).The percentages of CD4+and Treg were decreased(P<0.05),while those of CD8+and Th17 were increased(P<0.05).Compared to the model group,the probiotic and moxibustion groups showed lower Longa scores(P<0.01)and reduced cerebral infarct vol-umes(P<0.01).Tissue morphology in the ischemic brain and colon was relatively intact,and inflammatory cell infiltration was re-duced.The level of IL-10 was increased(P<0.01),IL-10 and FOXP3 mRNA,protein,and positive expressions were elevated(P<0.01).The level of IL-17A was decreased(P<0.01),and IL-17A and RORγt mRNA,protein,and positive expressions were reduced(P<0.01).The percentages of CD4+and Treg were increased(P<0.05),while those of CD8+and Th17 de-creased(P<0.05).Conclusion:Moxibustion ameliorate neurological function damage after ischemic stroke through brain-gut co-treatment,likely by regulating the Th17/Treg immune balance and inhibiting inflammatory responses in the brain-gut axis.
