复发性流产患者阴道分泌物的代谢组学分析OA

目的:探讨与复发性流产(recurrent spontaneous abortion,RSA)患者阴道分泌物相关的差异代谢物和代谢途径.方法:利用超高效液相色谱-四级杆-静电场轨道阱高分辨质谱联用(UPLC-Q-Exactive-MS)技术,对63例RSA患者和47例健康女性的阴道分泌物进行非靶向代谢组学分析,将多元和单元统计分析相结合,筛选两者的差异代谢物,并对差异代谢物进行代谢通路分析,进而寻找代谢物与RSA的相对关系.同时通过受试者工作特征(ROC)曲线分析差异代谢物对RSA的鉴别能力.结果:在健康非孕组和健康妊娠组中筛选出L-脯氨酸、L-亮氨酸等8种差异代谢物和精氨酸脯氨酸代谢等4条异常代谢通路.在RSA非孕组和健康非孕组中筛选出2,3-Dinor-8-表-前列腺素F2α、3-(2-萘基)-L-丙氨酸等13种差异代谢物和嘧啶代谢等2条异常代谢通路.在RSA妊娠组和健康妊娠组中筛选出甘氨熊脱氧胆酸、D-(-)-核糖等7种差异代谢物和磷酸戊糖途径等2条异常代谢通路.通过ROC曲线分析确定了曲线下面积(AUC)>0.7的差异性代谢物3种,包括2,3-Dinor-8-表-前列腺素F2α、3-(2-萘基)-L-丙氨酸、甘氨熊脱氧胆酸.结论:RSA患者与健康妇女的阴道分泌物代谢谱存在差异,2,3-Dinor-8-表-前列腺素F2α、3-(2-萘基)-L-丙氨酸、甘氨熊脱氧胆酸这3种代谢物对RSA表现了出色的判别效能,有望作为潜在的生物标志物.

Objective:To explore the differential metabolites and metabolic pathways associated with vaginal secretions in pa-tients with recurrent spontaneous abortion(RSA).Methods:Using UPLC-Q-Exactive-MS technology,we performed non-targeted metabolomics analysis on vaginal secretion samples from 63 patients with RSA and 47 healthy women,screened for differential metabolites between the two groups with combining multivariate and unitary statistical analysis,and the metabolic path-way analysis of the differential metabolite's was performed,thus finding the relative relationship between the metabolite and RSA.The discriminatory ability of differential metabolites to RSA was also analyzed by subject work characteristic curve(ROC).Re-sults:Eight differential metabolites,including L-proline and L-leucine,and four abnormal metabolic pathways,including arginine and proline metabolism,were screened in the healthy nonpregnant and healthy pregnant groups.Thirteen differential metabolites,including 2,3-Dinor-8-epi-prostaglandin F2α and 3-(2-Naphthyl)-L-alanine,and two abnormal metabolic pathways,including py-rimidine metabolism,were screened in the RSA nonpregnant and healthy nonpregnant groups.Seven differential metabolites,in-cluding glycoursodeoxycholic acid and D-(-)-Ribose,and two abnormal metabolic pathways,including the pentose phosphate pathway,were screened in the RSA pregnancy group and the healthy pregnancy group.Three differential metabolites with area un-der the curve(AUC)>0.7 were identified by ROC curve analysis,Including 2,3-Dinor-8-epi-prostaglandin F2α、3-(2-Naph-thyl)-L-alanine and glycoursodeoxycholic acid.Conclusion:There are differences in the metabolic profiles of vaginal secretions be-tween RSA patients and healthy women.Three metabolites,2,3-Dinor-8-epi-prostaglandin F2α,3-(2-Naphthyl)-L-alanine and glycoursodeoxycholic acid showed excellent discriminatory efficacy for RSA and are expected to serve as potential biomarkers.
