基于"伏邪"理论探讨类风湿关节炎的态靶辨治OA

Exploration of the syndrome-target differentiation and treatment of rheumatoid arthritis based on the theory of"latent pathogens"

类风湿关节炎(RA)是一种自身免疫性炎性疾病,可归属于中医学"痹证"范畴,其导致的关节疼痛、骨破坏及关节外多系统损害对患者健康造成严重危害.本团队根据该病发病特点,以"伏邪"理论结合态靶辨治理论,将RA分为冒邪、邪张、邪潜、邪痼四期,剖析病机演变,锚定其反复发作、难以根治的治疗难点,为RA整体辨治提供新思路,形成扶正气、散寒湿、清瘀热、通经络的RA治疗总则.本团队将RA临床表现特征归纳为虚、寒、湿、热、瘀五态,以桂枝芍药知母汤为病靶主方,在分期化裁的基础上,结合患者最突出的一态加减,并加入针对症状、理化指标、影像靶点的靶药,构建分期辨态打靶的RA辨治体系以提高中医药辨治RA临床水平.

Rheumatoid arthritis(RA)is an autoimmune inflammatory disease that can be categorized under the traditional Chinese medicine(TCM)concept of"Bi syndrome."It causes joint pain,bone destruction,and extrasynovial multisystem damage,posing severe threats to patients' health.Based on the disease's onset characteristics,we illustrate the theory of"latent pathogens"combined with the syndrome-target differentiation and treatment concept,divide the disease into four stages:emergence,expansion,latency,and chronicity.This approach aligns with the evolution of pathogenesis,identify the challenge of recurrent episodes and difficulty in achieving a cure,and provide new ideas for the holistic differentation and treatment of RA,forming a general principle of strengthening the vital qi,dispelling cold and dampness,clearing blood stasis and heat,and unblocking meridians for RA treatment.In terms of disease manifestations,five states of"deficiency,cold,dampness,heat,and stasis"are summarized.With Guizhi Shaoyao Zhimu Decoction as the main prescription targeting the disease,staging-based modification as the foundation,adjustments needed to be made according to the patient's most prominent state,and incorporation of target drugs aimed at symptoms,physicochemical indicator,and imaging targets.RA treatment system based on staged differentiation and targeted therapy is constructed to improve the clinical effecacy.
