经颅重复磁刺激配合头皮针在脑卒中后认知合并吞咽障碍患者的疗效研究OA
Study on The Efficacy of Transcranial Magnetic Stimulation Combined with Scalp Acupuncture in Patients with Cognitive and Swallowing Disorders after Stroke
目的:探讨经颅重复磁刺激配合头皮针对脑卒中后认知合并吞咽障碍患者的疗效.方法:选取2022年7月～2024年7月就诊本院脑卒中后认知合并吞咽障碍患者60例,采用随机数字表法分为对照组与联合组各30例.对照组采用头皮针+常规康复治疗,联合组在对照组基础上增加经颅重复磁刺激治疗,于治疗前及4周治疗后比较两组患者认知功能(MMSE评分、MoCA评分)及吞咽功能(口腔运送时间OTT、VDS评分).结果:治疗后,两组患者MMSE、MoCA评分均较治疗前升高(P<0.01),且观察组治疗后MMSE、MoCA评分显著高于对照组(P<0.01).治疗后,两组患者OTT均较治疗前缩短(P<0.01),对照组VDS评分较治疗前无统计学差异(P>0.05),联合组VDS评分较治疗前降低(P<0.01),且观察组治疗后OTT、VDS评分均优于对照组(P<0.01).结论:经颅重复磁刺激治疗联合头皮针治疗可以更好地改善脑卒中患者认知、吞咽功能.
Objective:To investigate the efficacy of transcranial magnetic stimulation(TMS)combined with scalp acupuncture in patients with cognitive and swallowing disorders after stroke.Methods:Sixty patients with post-stroke cognitive and swallowing disorders admitted to our hospital from July 2022 to July 2024 were selected and randomly divided into a control group and a combination group,with 30 cases in each group,using a random number table method.The control group received scalp acupuncture and conventional rehabilitation treatment,while the combination group was additionally given repetitive transcranial magnetic stimulation(rTMS)on the basis of the control group's treatment.Cognitive function[Mini-Mental State Examination(MMSE)score,Montreal Cognitive Assessment(MoCA)score]and swallowing function[oral transit time(OTT),Video Fluoroscopic Dysphagia Scale(VDS)score]of the two groups were compared before and after 4 weeks of treatment.Results:After treatment,the MMSE and MoCA scores of both groups were significantly higher than those before treatment(P<0.01),and the MMSE and MoCA scores of the combination group were significantly higher than those of the control group(P<0.01).After treatment,the OTT of both groups was significantly shortened compared with that before treatment(P<0.01),while there was no statistically significant difference in the VDS score of the control group compared with that before treatment(P>0.05).The VDS score of the combination group was significantly lower than that before treatment(P<0.01),and the OTT and VDS score of the combination group were significantly better than those of the control group after treatment(P<0.01).Conclusion:Transcranial magnetic stimulation combined with scalp acupuncture can better improve the cognitive function and swallowing function of patients with cognitive and swallowing disorders after stroke.
