Different Interventions on Neurogenic Intestinal Dysfunction in Patients with Spinal Cord Injury:A Network Meta-Analysis

目的:旨在系统评估不同康复疗法对脊髓损伤患者神经源性肠道功能障碍的治疗效果,以提供临床实践的循证干预方案.方法:通过检索知网、维普、万方、中国生物医学文献、PubMed、Cochrane Library、Web of Science、Embase和Scopus数据库,搜索自建库起至2024年1月1日为止的关于脊髓损伤患者接受不同康复疗法的随机对照试验,采用RevMan5.3和Stata 14.2软件执行网状Meta分析.结果:共纳入48项研究,涉及8种不同的干预方法,总计3269名脊髓损伤并伴有神经源性肠道功能障碍的患者.网状Meta分析表明,直肠球囊扩张术和电刺激在改善患者神经源性肠道功能障碍评分方面效果最佳,而电刺激在提高患者排便有效率方面表现最佳.结论:直肠球囊扩张术和电刺激在改善脊髓损伤患者神经源性肠道功能障碍方面效果最为突出,建议在临床实践中优先考虑这两种疗法,然而仍需更多高质量的随机对照试验来进一步验证.

Objectives:To assess the effectiveness of various rehabilitation therapies in treating neurogenic intestinal dysfunction(NID)in patients with spinal cord injury(SCI),so as to provide evidence-based interventions for clinical practice.Methods:Randomized controlled trials(RCTs)on different rehabilitation therapies for patients with SCI complicated with NID were retrieved from CNKI,VIP,Wanfang,Chinese Biomedical Literature Database(CBM),PubMed,the Cochrane Library,Web of Science,Embase,and Scopus databases,with the search period from the establishment of each database to January 1,2024.Network Meta-analysis was performed using RevMan 5.3 and Stata 14.2 software.Results:A total of 48 studies involving 3269 SCI patients with neurogenic bowel dysfunction(NBD)were included,covering 8 intervention methods.The network Meta-analysis results showed that rectal balloon dilatation and electrical stimulation were the most effective interventions in improving the total score of NBD patients.In addition,electrical stimulation was found to be the most effective in improving defecation efficiency.Conclusion:Rectal balloon dilatation and electrical stimulation are the most effective interventions for improving neurogenic intestinal dysfunction in patients with spinal cord injury.
