电针夹脊穴联合筋膜刀治疗飞行人员腰肌劳损的临床研究OA
Clinical Study on Electroacupuncture at Jiaji Acupoint Combined with Fascial Knife for the Treatment of Lumbar Muscle Strain in Pilots
目的:探析电针夹脊穴联合筋膜刀治疗飞行人员腰肌劳损的效果.方法:选择 2021 年 9 月—2024 年 8 月期间样本医院接诊的 80 例飞行人员腰肌劳损患者,按随机数表法分为观察组和对照组,每组各 40 例.两组均接受一般治疗,在此基础上对照组予电针夹脊穴治疗,观察组予电针夹脊穴联合筋膜刀治疗,两组均治疗 4 周.比较两组疗效,治疗前后疼痛情况[采用疼痛数字评价量表(NRS)评估]、竖脊肌表面肌电信号、腰椎功能、生活质量,治疗后随访 6 个月,统计复发情况.结果:观察组治疗总有效率高于对照组,且观察组整体疼痛程度低于对照组,差异有统计学意义(P<0.05);两组平均肌电波幅(AEMG)、平均功率频率(MPF)水平、健康状况调查问卷(SF-36)各维度评分均高于治疗前,且观察组AEMG、MPF水平、SF-36 各维度评分均高于对照组,差异有统计学意义(P<0.05);两组Oswestry功能障碍指数(ODI)评分均低于治疗前,且观察组ODI评分低于对照组,差异有统计学意义(P<0.05);随访期间,观察组和对照组的复发率比较,差异无统计学意义(P>0.05).结论:电针夹脊穴联合筋膜刀治疗飞行人员腰肌劳损,能增强镇痛效果,显著改善患者竖脊肌肌力、腰椎功能及生活质量.
Objective:To exploring the effect of electroacupuncture at Jiaji acupoint combined with fascial knife in treating lumbar muscle strain in pilots.Method:80 patients with lumbar muscle strain in pilots who were treated between September 2021 and August 2024 were selected and divided into an observation group and a control group using a random number table method,with 40 patients in each group.Both groups of patients received general treatment.On this basis,the control group was treated with electroacupuncture at Jiaji acupoint,while the observation group was treated with electroacupuncture at Jiaji acupoint combined with fascial knife.Both groups were treated for 4 weeks.The therapeutic effects of the two groups were compared.The pain conditions before and after treatment[evaluated by the Numerical Rating Scale for Pain(NRS)],the surface electromyography signals of the erector spinae muscles,lumbar function,and quality of life were examined.The patients were followed up for 6 months after treatment,and the recurrence was statistically analyzed.Result:The total effective rate of the observation group was higher than that of the control group,and the overall pain level of the observation group was lower than that of the control group,with statistical significant differences(P<0.05).The average electromyographic amplitude(AEMG),average power frequency(MPF)levels,and SF-36 scores in both groups were higher than before treatment,and the observation group had higher AEMG,MPF levels,and SF-36 scores in all dimensions than the control group,with statistical significant differences(P<0.05).The Oswestry Disability Index(ODI)scores of both groups were lower than before treatment,and the ODI scores of the observation group were lower than those of the control group,with statistical significant differences(P<0.05).During the follow-up period,there was no statistical significant difference in the recurrence rate between the observation group and the control group(P>0.05).Conclusion:The combination of electroacupuncture at Jiaji acupoint and fascial knife can enhance the analgesic effect and significantly improve the vertical spine muscle strength,lumbar spine function,and quality of life of pilots with lumbar muscle strain.
