基因标志物评分预测关节镜下部分半月板切除术后骨关节炎风险OA
Gene marker score predicts the risk of osteoarthritis after arthroscopic partial meniscectomy
目的 研究关节镜下部分半月板切除术(APM)后骨关节炎(OA)患者血浆mRNA表达,并构建基因标志物评分模型评估OA风险.方法 选取2019年1月至2021年12月间我院收治的268例APM术后患者,按倾向性评分匹配分为OA组与对照组,每组78例.取每组6例血浆样本进行mRNA微阵列分析,通过京都基因与基因组数据库、蛋白质-蛋白质相互作用(PPI)网络筛选枢纽基因,并采用实时定量PCR、ELISA进行验证.采用多因素logistic回归分析OA风险,通过受试者操作特征(ROC)曲线评估变量的预测性能.结果 268例APM术后患者中,OA发生率为29.1%(78/268).共筛选出171个差异基因,功能富集于细胞外基质、FoxO等通路;PPI网络鉴定出COL1A1、COL3A1、HLA-DRA、PXDN等7个枢纽基因.验证显示COL1A1、COL3A1、HLA-DRA、PXDN基因在OA组显著高表达(P<0.05).基于其构建的基因标志物评分能独立预测OA风险(P<0.05),ROC曲线下面积达0.931(0.879~0.965).结论 COL1A1、COL3A1、HLA-DRA与PXDN基因参与APM术后OA发病,基于此构建的基因标志物评分可有效预测OA风险,有助于识别高危患者.
Objective To explore the expression of plasma mRNA in patients with osteoarthritis(OA)after arthroscopic partial menis-cectomy(APM),and construct a gene marker scoring model to assess OA risk.Methods A total of 268 patients who underwent APM at our hospital between January 2019 and December 2021 were included in this study.Using 1∶1 propensity score matching,78 patients were allocated to the OA group and 78 to the control group.Plasma samples from six participants in each group were subjected to mRNA microarray analysis.The hub genes were identified using KEGG pathway and protein-protein interaction(PPI)network analyses,and their expression was validated using real-time PCR,Western blotting(WB),and ELISA.Multivariate logistic regression was used to analyze the OA risk and the predictive performance of the variables was evaluated using receiver operating characteristic(ROC)curves.Results The incidence of OA was 29.1%(78/268).mRNA analysis identified 171 differentially expressed genes that were functionally enriched in pathways,such as extracellular matrix organization and FoxO signaling.PPI network analysis identified seven hub genes including COL1A1,COL3A1,HLA-DRA,and PXDN.Multi-technique validation confirmed that these four genes were significantly upregulated in the OA group(P<0.05).A gene marker score constructed based on these genes independently predicted OA risk in the multivariate ana-lysis(P<0.05),with an area under the curve of 0.931(0.879-0.965).Conclusion COL1A1,COL3A1,HLA-DRA,and PXDN have been implicated in OA pathogenesis.A gene signature score constructed based on these genes effectively predicted the OA risk and aided in the identification of high-risk patients.
