目的:研究不同电针波形对因脑小血管病(CSVD)引发的膀胱过度活动症(OAB)患者尿动力学的影响.方法:选取2022年1月—2024年3月武汉市中医医院康复科、脑病科的住院患者及门诊70例脑小血管病所致膀胱过度活动症患者,按照随机数字表法将其分为电针连续波组和电针疏密波组,每组35例.两组的取穴方法相同,均取双侧足运感区、双侧中髎及会阳穴治疗,电针分别采用连续波和疏密波正负极分别连接于同侧的中髎穴和会阳穴,1次/d,6次/周,共治疗4周.观察治疗前后两组患者的膀胱过度活动症评分(OABSS)、膀胱过度活动症生活质量调查表(OAB-q)及尿动力学指标,包括急迫尿意容量(VUD)、初始尿意容量(FDV)和最大尿流率(MFR)的变化.结果:两组治疗后OABSS评分和OAB-q评分均显著低于治疗前,差异具有统计学意义(P<0.01),其中疏密波组的OABSS评分和OAB-q评分的降低程度显著优于连续波组,差异具有统计学意义(P<0.01).治疗后两组FDV和MFR均较治疗前显著升高,差异具有统计学意义(P<0.05),而PVR在治疗后均显著降低,差异具有统计学意义(P<0.05);治疗后,疏密波组的FDV和MFR均高于连续波组,差异具有统计学意义(P<0.05),PVR则低于连续波组,差异具有统计学意义(P<0.05).结论:电针疏密波和电针连续波两种治疗方法均能够有效改善因脑小血管病引起的OAB患者的临床排尿状况及生活质量,且电针疏密波组显示出更明显的优势效果.

Objective:To investigate the effects of different electro-acupuncture(EA)wave forms on urodynamic parameters in patients with overactive bladder(OAB)caused by cerebral small vessel disease(CSVD).Methods:A total of 70 patients with OAB caused by CSVD were randomly assigned into the EA A group and the EA B group,with 35 cases in each group.The EA A group was given continuous wave EA treatment,and the EA B group was given dense-disperse wave EA treatment.Both groups received electro-needling the same acupoints,including bilateral foot sensory areas,bilateral Zhongliao(BL33),and Huiyang(BL35).In both groups,the positive and negative electrodes were connected respectively to BL33 and BL35 on the same side,once daily for 6 consecutive days followed by a one-day rest,for a total duration of 4 weeks.Scores of Overactive Bladder Symptom Scale(OABSS)and Overactive Bladder Questionnaire(OAB-q)for quality of life,as well as urodynamic parameters,including volume at urgent desire(VUD),first desire to void(FDV),and maximum flow rate(MFR)were assessed before and after the treatment in the two groups.Results:The scores of OABSS and OAB-q were significantly lower after the treatment than those before the treatment in the two groups(P<0.01),and the scores were lower in the EA B group than those in the EA A group(P<0.01).The values of FDV and MFR were significantly higher(P<0.05),whereas the value of PVR was significantly lower after the treatment than before the treatment in the two groups(P<0.05);the values of FDV and MFR were higher(P<0.05),and the value of PVR was lower in the EA B group than the EA A group(P<0.05).Conclusion:Both dense-disperse wave EA and continuous wave EA effectively improve urination function and quality of life in patients with OAB caused by CSVD.Moreover,the dense-disperse wave EA demonstrates a more pronounced therapeutic advantage compared with the continuous wave EA.
