超声引导小针刀联合隔姜灸治疗成人屈指肌肌腱狭窄性腱鞘炎对患指功能的改善作用OA
Effect of Ultrasound-Guided Acupotomy Combined with Ginger-Separated Moxibustion on Affected Finger Function in Adults with Stenosing Tenosynovitis of Flexor Tendon
目的:探究超声引导小针刀联合隔姜灸治疗成人屈指肌肌腱狭窄性腱鞘炎对患指功能的改善作用.方法:采用随机数字表法将2023年4月—2024年4月在联勤保障部队第九八○医院和邯郸二八五医院接受治疗的86例成人屈指肌肌腱狭窄性腱鞘炎患者分为对照组43例与治疗组43例.对照组接受腱鞘内注药治疗,治疗组在腱鞘内注药的基础上行超声引导小针刀联合隔姜灸治疗,治疗后随访3个月.比较两组治疗后3个月的疗效,治疗前、治疗3个月后的患指功能、疼痛、关节结构、活动度、负性情绪以及生活质量,随访期间的不良反应.结果:治疗组总有效率为97.67％(42/43),高于对照组的81.40％(35/43),差异具有统计学意义(P＜0.05).治疗后,两组Quinnell、视觉模拟评分(VAS)降低,且治疗组更低,差异具有统计学意义(P＜0.05).治疗后,两组腱鞘厚度、肌腱厚度降低,且治疗组中更低,差异具有统计学意义(P＜0.05);治疗后,两组关节活动度升高,且治疗组更高,差异具有统计学意义(P＜0.05).治疗后,两组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分降低,且治疗组更低,差异具有统计学意义(P＜0.05);治疗后,两组健康调查简表(SF-36)评分升高,且治疗组更高,差异具有统计学意义(P＜0.05).治疗过程中,治疗组不良反应发生率13.95％(6/43)、对照组9.30％(4/43),两组比较,差异无统计学意义(P＞0.05).结论:超声引导小针刀联合隔姜灸治疗成人屈指肌肌腱狭窄性腱鞘炎,可改善患指功能,缓解疼痛,调节关节结构、活动度,降低负性情绪,提高生活质量,安全性良好.
Objective:To investigate the effect of ultrasound-guided acupotomy combined with ginger-separated moxibustion on improving affected finger function in adults with stenosing tenosynovitis of flexor tendon.Methods:A total of 86 adult patients with stenosing tenosynovitis of flexor tendon were randomly assigned into the control group(n=43)and the treatment group(n=43).The control group received local injection therapy into tendon sheath;on which basis,the treatment group also received ultrasound-guided acupotomy combined with ginger-separated moxibustion.Patients were followed up for 3 months after the treatment.The two groups were compared in terms of clinical efficacy at 3 months post-treatment,affected finger function,pain,joint structure,range of motion,negative emotions,and quality of life before the treatment and at 3 months after the treatment,as well as adverse reactions during the follow-up period.Results:The total effective rate was 97.67％(42/43)in the treatment group,which was significantly higher than 81.40％(35/43)in the control group(P＜0.05).After treatment,Quinnell scores and VAS scores decreased in both groups after the treatment,with significantly lower scores observed in the treatment group compared to the control group(P＜0.05).Tendon sheath thickness and tendon thickness were reduced in both groups after the treatment,with the treatment group showing greater reductions(P＜0.05).Joint range of motion increased in both groups,with a greater increase in the treatment group(P＜0.05).The scores of SAS and SDS decreased in both groups after the treatment,with lower scores in the treatment group(P＜0.05).Health-related quality of life(SF-36)scores increased in both groups,with higher scores in the treatment group(P＜0.05).During treatment,the incidence of adverse reactions was 13.95％(6/43)in the treatment group and 9.30％(4/43)in the control group,in which there was no statistical difference between the two groups(P＞0.05).Conclusion:Ultrasound-guided acupotomy combined with ginger-separated moxibustion can improve finger function,relieve pain,regulate joint structure and mobility,reduce negative emotions,and enhance quality of life in adults with stenosing tenosynovitis of flexor tendon.The treatment demonstrates good safety.
