调督理筋针法联合三步推拿法治疗腰椎间盘突出症患者的临床研究OA
Clinical Study on Tiaodu Lijin Acupuncture Combined with Three-Step Tuina Manipulation in Treatment of LDH
目的:探讨调督理筋针法联合三步推拿法治疗腰椎间盘突出症(LDH)的疗效.方法:纳入本院2023年5月—2024年5月收治的LDH患者86例,采用随机数字表法分为两组,观察组、对照组各43例,治疗过程中对照组脱落1例,观察组脱落5例,最终对照组42例、观察组38例.对照组给予三步推拿法,观察组在对照组基础上联合调督理筋针法.比较两组患者的治疗效果,治疗前后采用日本骨科协会颈髓功能评分(JOA评分)和视觉模拟评分法(VAS)评估患者的腰椎功能和疼痛程度,采用世界卫生组织生存质量量表简表(WHOQOL-BREF)评价患者的生活质量,采用直腿抬高试验阳性率和腰背肌后伸关节活动度(ROM)评估脊柱的活动度和稳定性.比较两组治疗前后的中医症候积分、炎性因子水平.结果:观察组总有效率94.74％(36/38),高于对照组的76.19％(32/42),差异具有统计学意义(P＜0.05).治疗后,两组JOA和VAS评分改善,生活质量提高,直腿抬高试验阳性率降低,腰背肌后伸ROM提升,中医症候积分和炎性因子水平降低,这些指标观察组更优,差异具有统计学意义(P＜0.05).结论:调督理筋针法联合三步推拿法治疗LDH具有较好的疗效,可改善腰部功能,能够有效缓解疼痛,明显提高患者生活质量.
Objective:To investigate the therapeutic effect of Tiaodu Lijin(Du meridian-regulating and muscle-managing)acupuncture combined with three-step Tuina manipulation in treatment of lumbar disc herniation(LDH).Methods:A total of 86 patients with LDH were randomly divided into the observation group and the control group,with 43 patients in each group.During treatment,one patient in the control group and five patients in the observation group withdrew,resulting in 38 cases in the observation group and 42 cases in the control group being included in the final analysis.The control group received treatment with three-step Tuina manipulation;on which basis,the observation group was also treated with Tiaodu Lijin acupuncture.The therapeutic effects of the two groups were compared.Lumbar function and pain intensity were evaluated before and after the treatment using the scores of JOA and VAS.Quality of life was assessed using WHOQOL-BREF.Spinal mobility and stability were evaluated through the positive rate of the straight leg raising test and the ROM of lumbar back extension.In addition,changes in TCM symptom scores and inflammatory factor levels were compared between the two groups before and after the treatment.Results:The total effective rate in the observation group was 94.74％(36/38),which was significantly higher than 76.19(32/42)in the control group(P＜0.05).After the treatment,both groups showed improvements in the scores of JOA and VAS,enhanced quality of life,a decreased positive rate of the straight leg raising test,increased lumbar back extension ROM,and reductions in TCM symptom scores and inflammatory factor levels.All these improvements were significantly greater in the observation group compared with the control group(P＜0.05).Conclusion:The combination of Tiaodu Lijin acupuncture and three-step Tuina manipulation demonstrates good therapeutic efficacy in treatment of LDH.This combined approach can improve lumbar function,effectively relieve pain,and significantly enhance patients' quality of life.
