Joint developmental trajectories of depression and suicidality in Chinese college students and their relationship with adverse childhood experiences

目的:探讨大学生抑郁与自杀倾向的联合发展轨迹及童年期不良经历的影响.方法:采用纵向设计对 3 797 名大学生进行 4 年追踪,运用贝克抑郁量表(BDI-Ⅱ-C)、青少年自杀倾向问卷(ASTS)、童年期不良经历问卷(ACEs)和症状自评量表(SCL-90)对大学生的抑郁、自杀倾向、童年不良成长经历和心理健康症状进行评估.通过潜类别增长模型(LCGM)识别抑郁和自杀倾向的联合发展轨迹,采用方差分析检验不同亚组的心理健康差异,多分类logistic回归分析ACEs的预测效应.结果:抑郁与自杀倾向存在4 类联合发展轨迹,即低抑郁-低自杀-上升组(C1,26.3%)、中抑郁-高自杀-上升组(C2,11.6%)、高抑郁-高自杀-速降组(C3,10.0%)及低抑郁-低自杀-缓降组(C4,52.1%).ACEs中高虐待、高忽视个体更易发展为C1 和C2 亚组,且男生更易进入C2 亚组(OR=0.64).心理健康症状 10 个因子的严重程度呈C3=C2>C1>C4 梯度.结论:本研究识别出4 类抑郁与自杀倾向联合发展轨迹,"高抑郁-高自杀-速降组"和"中抑郁-高自杀-上升组"表现出较高心理健康风险特征,且ACEs中的虐待和忽视是重要的风险因素.

Objective:To identify joint developmental trajectories of depression and suicidality among college students and examine the role of adverse childhood experiences(ACEs).Methods:Using a longitudinal design,3 797 college students were followed up for 4 years.The Beck Depression Inventory-Ⅱ-Chinese(BDI-Ⅱ-C),Ado-lescent Suicide Tendency Scale(ASTS),Adverse Childhood Experience questionnaire(ACEs)and Symptom Checklist 90(SCL-90)were used to assess the depression,suicidality,adverse childhood experiences and mental health symptoms of college students.Latent class growth modeling(LCGM)was used to identify the joint develop-mental trajectories of depression and suicidality.Variance analysis was used to test the differences in mental health among different subgroups.Multinomial logistic regression analysis was used to test the predictive effect of ACEs.Results:Four distinct trajectories were identified,namely low depression-low suicidality-increasing group(C1,26.3%),moderate depression-high suicidality-increasing group(C2,11.6%),high depression-high suicidali-ty-rapidly decreasing group(C3,10.0%)and low depression-low suicide-slow decreasing group(C4,52.1%).Individuals with high abuse and high neglect in ACEs were more likely to be in the group C1 and C2,and males were more likely to be in group C2(OR=0.64).The severity of 10 factors of mental health symptoms followed gradient C3=C2>C1>C4.Conclusion:This study identifies four joint trajectories of depression and suicidality in college students,highlights the heightened psychological risks in certain subgroups,and confirms abuse and neglect in childhood as key risk factors.
