免疫检查点抑制剂在乳腺癌治疗中展现出显著的临床获益;但其疗效受限于肿瘤微环境(TME)介导的免疫治疗耐药.TME中的免疫抑制性细胞,如调节性T细胞、肿瘤相关巨噬细胞和髓系抑制性细胞等通过多种机制导致免疫治疗耐药.此外,癌症相关成纤维细胞及肿瘤区域的低氧环境亦对免疫治疗效果产生不利影响.文章系统性阐述调控TME以克服免疫耐药的最新研究进展,涵盖针对免疫抑制细胞的精准靶向、代谢调控、光疗、肿瘤免疫基因调控以及联合疗法与纳米颗粒介导的治疗策略等方面,为进一步提高乳腺癌免疫治疗的整体疗效提供参考.

While immune checkpoint inhibitors have shown substantial clinical benefits in treating breast cancer,their overall efficacy is limited by resistance mechanisms driven by the tumor microenvironment(TME).Notably,various immunosuppressive cell populations—such as regulatory T cells,tumor-associated macrophages,and myeloid-derived suppressor cells—play a crucial role in fostering therapeutic resistance through multiple pathways.In addition,cancer-associated fibroblasts and the hypoxic conditions characteristic of tumor regions further diminish the effectiveness of immunotherapy.This review offers a comprehensive examination of the latest strategies to modulate the TME and overcome immune resistance.The discussion spans targeted approaches against immunosuppressive cells,metabolic regulation,phototherapy,modulation of tumor immunogenetics,as well as combination therapies and nanoparticle-mediated treatment strategies.Collectively,these developments provide novel theoretical insights and practical avenues for enhancing the overall efficacy of immunotherapeutic interventions in breast cancer.
