目的 探讨动态对比增强磁共振成像(DCE-MRI)影像组学特征对浸润性乳腺癌前哨淋巴结转移负荷的预测价值.方法 纳入 2022 年 12 月至 2024 年 12 月在常德市第一人民医院接受治疗的 186 例乳腺癌患者,并采用分层抽样的方式按照 7∶3比例分为训练集(n=130)和验证集(n=56).根据患者术后的病理淋巴结转移状态,进一步分为高转移负荷组(n=99)和低转移负荷组(n=87).LASSO回归方法进行组学特征的筛选,多因素Logistic分析前哨淋巴结转移的影响因素,并建立前哨淋巴结转移的预测模型,评价各特征的预测效能.结果 高转移负荷组和低转移负荷组患者年龄、肿瘤家族史和病理类型等差异有统计学意义(P<0.05).DCE-MRI影像资料结果显示,高转移负荷组患者肿瘤最大径、分叶比例、不规则形状比例、不清晰边界比例、背景实质强化明显比例和弹性应变率等差异有统计学意义(P<0.05).多因素分析结果显示,年龄、肿瘤家族史、病理类型、肿瘤最大径、有无分叶、形状、边界、背景实质强化、ADC值、弹性应变率、DCE小波HH域灰度游程矩阵-高灰度级短游程强调、小波LH方向一阶分解的灰度共生矩阵-信息度量相关 1、原始图像形状特征-最小轴长度、原始图像形状特征-长轴长度、DCE指数滤波邻域灰度差矩阵-复杂度和DCE小波LH域灰度共生矩阵-集群阴影是影响前哨淋巴结转移的独立影响因素.基于此建立的预测模型灵敏度为 76.8%,特异度为 83.6%.依据影响因素构建的预测模型显示,内部验证前后AUC分别为0.853(95%CI:0.792~0.897)和0.867(95%CI:0.819~0.907),灵敏度分别为 86.93%和 87.89%,特异度分别为 91.05%和 93.82%.结论 DCE-MRI影像组学特征在预测浸润性乳腺癌前哨淋巴结转移负荷方面具有重要的应用价值.该法可为临床诊断、后期治疗及预后评估提供参考信息.

Objective To explore the predictive value of DCE-MRI radiomics features for sentinel lymph node(SLN)metastasis burden in invasive breast cancer.Methods From December 2022 to December 2024,186 breast cancer patients treated in our hospital were stratified and divided into a training set(n=130)and a validation set(n=56)in a 7∶3 ratio.Based on postoperative pathological lymph node metastasis status,the patients were further stratified into a high metastatic burden group(n=99)and a low metastatic burden group(n=87).Least Absolute Shrinkage and Selection Operator(LASSO)regression was employed to screen omics features.Multivariable Logistic analysis was conducted to investigate factors influencing sentinel lymph node metastasis,and a predictive model for sentinel lymph node metastasis was established to evaluate the predictive performance of each feature.Results Significant differences were observed between the high metastatic burden groupand the low metastatic burden group in terms of patient age,family history of cancer,and pathological type(P<0.05).DCE-MRI imaging data revealed statistically significant differences in the maximum tumor diameter,the proportion with lobulation,the proportion with irregular shapes,the proportion with unclear boundaries,the proportion with obvious background parenchymal enhancement,and the elastic strain rate in the high metastatic burden group(P<0.05).Multivariate analysis indicated that age,family history of cancer,pathological type,maximum tumor diameter,presence of lobulation,shape,boundary,background parenchymal enhancement,ADC value,elastic strain rate,DCE_wavelet-HH_glrim_ShortRunHighGrayLevelEmphasis,wavelet_glcm_wavelet-1lh-imc1,original_shape_leastaxislength,original_shape_majoraxislength,DCE_exponential_ngtdm_Complexity,and DCE_wavelet-LH_glcm_Clustershade were independent factors influencing sentinel lymph node metastasis.The predictive model established based on these factors had a sensitivity of 76.8%and a specificity of 83.6%on the Receiver Operating Characteristic(ROC)curve.The prediction model constructed based on influencing factors demonstrated an AUC of 0.853(95%CI:0.792-0.897)before internal validation and 0.867(95%CI:0.819-0.907)after validation,with sensitivities of 86.93%and 87.89%,and specificities of 91.05%and 93.82%,respectively.Conclusions DCE-MRI radiomics features have significant application value in predicting SLN metastasis burden in invasive breast cancer.This method can provide reference information for clinical diagnosis,subsequent treatment,and prognosis assessment.
