环泊酚用于全身麻醉或镇静患者的疗效及安全性的系统评价再评价OA
Overview of Systematic Evaluation on Efficacy and Safety of Ciprofol for General Anesthesia or Sedation:Re-Assessment of Systematic Review
目的:通过系统评价再评价方法,汇总并评估现有比较环泊酚与丙泊酚的系统评价/Meta分析(SR/MA)证据,为临床用药提供循证依据.方法:系统检索中英文数据库,包括PubMed、Embase、the Cochrane Library、中国知网等,纳入比较环泊酚与丙泊酚用于全身麻醉或镇静的SR/MA(检索时限为建库至 2024 年 11 月 30 日).采用系统评价方法学质量评价工具(AMSTAR 2)评估方法学质量,采用推荐分级的评估、制订与评价(GRADE)工具系统评价证据等级,并计算原始研究重叠程度,汇总核心结局指标.结果:共纳入 15 篇SR/MA,AMSTAR 2 方法学质量评价显示,3 篇为高质量文献,其余文献多数存在关键条目缺陷;GRADE评价显示,大部分文献中有效性和安全性证据质量为低或极低,重叠分析表明原始研究存在高度重复(修正重叠区域=0.17).评价结果显示,环泊酚与丙泊酚在镇静成功率等有效性结局方面效果相当,环泊酚在减轻注射疼痛、提升患者满意度及改善呼吸安全性方面表现出一定优势,部分指标证据质量较高.血流动力学事件与术中体动控制等方面的证据结果不一致,尚缺乏高质量证据支持.结论:现有证据提示,用于全身麻醉或镇静患者时,环泊酚在部分安全性指标方面优于丙泊酚,二者疗效相当.但研究质量整体较低,且地区集中,限制了证据的外部推广性.需开展高质量原始研究和SR,提升证据可靠性和临床指导价值.
OBJECTIVE:To summarize and evaluate the existing systematic reviews/Meta-analyses(SR/MA)evidence for comparison of ciprofol and propofol through the re-assessment of systematic review methods,so as to provide evidence-based guidance for clinical medication.METHODS:English and Chinese databases such as PubMed,Embase,the Cochrane Library,and CNKI were systematically searched to identify relevant SR/MA of comparison of ciprofol and propofol for general anesthesia or sedation(with a search period from database establishment to Nov.30th,2024).Methodological quality was assessed by using A Measurement Tool to Assess Systematic Reviews(AMSTAR 2),evidence certainty was graded via the Grading of Recommendations Assessment,Development and Evaluation(GRADE)tool.Overlap of primary studies was calculated,and core outcome measures were summarized.RESULTS:Fifteen SR/MA were included.Methodological quality assessment of AMSTAR 2 revealed 3 high-quality literature,while most literature had critical flaws.GRADE evaluation indicated low or very low certainty for most efficacy and safety evidence.Overlap analysis demonstrated high redundancy among primary studies(correct overlapping areas=0.17).Results showed comparable efficacy between ciprofol and propofol in sedation success rates,with ciprofol exhibiting advantages in reducing injection pain,improving patient satisfaction,and enhancing respiratory safety,partial indicators had relatively high evidence quality.Inconsistent findings were observed in hemodynamic events and intraoperative movement control,lacking robust evidence.CONCLUSIONS:Current evidence suggests that ciprofol offers superior safety in specific aspects compared with propofol while maintaining equivalent efficacy for patients undergoing general anesthesia or sedation.However,the overall low quality of included studies and regional concentration limit the generalizability of findings.High-quality primary research and SR are warranted to enhance evidence reliability and clinical applicability.
