治疗心系疾病的中药用药规律分析及药学监护探讨OA
Analysis on Medication Rules and Pharmaceutical Care of Traditional Chinese Medicine in the Treatment of Cardiovascular Diseases
目的:基于数据挖掘技术分析中药治疗心系疾病的用药规律,并探讨相关药学监护要点.方法:建立治疗心系疾病的中药处方数据库,应用古今医案云平台对 2022 年 1 月至 2023 年 6 月该院住院患者的中药处方进行数据挖掘,分析治疗心系疾病的用药规律,并对高频中药和高风险中药进行药学监护点总结.结果:共收集中药处方 3 741 张,所用中药的药性以平、温为主,五味以甘、苦为主,归经以肺、肝为主.使用频次排序居前 30 位的中药中,平均剂量超《中华人民共和国药典》或《北京市中药饮片炮制规范》用量的中药有 23 种.关联规则分析得到,支持度、共现度最高的药对分别为红花-桃仁、茯苓-党参.聚类分析发现4组核心药物组合,(1)茯苓、丹参、党参、黄芪;(2)柴胡、黄芩;(3)白术、法半夏、陈皮;(4)当归、川芎、牛膝、赤芍、桃仁、红花.结论:心系疾病临床用药过程中加强对中西药合用及不良反应的监测,可为心内科临床中药师提供药物警戒监护要点及参考.
OBJECTIVE:To analyze the medication rules of traditional Chinese medicine(TCM)in the treatment of cardiovascular diseases based on data mining technology,and to explore key pharmaceutical care points.METHODS:A database of TCM prescriptions for cardiovascular diseases was established.Data mining was performed on TCM prescriptions for inpatients from Jan.2022 to Jun.2023 by using the Cloud Platform for Medical Cases to analyze the medication patterns.The medication patterns in the treatment of cardiovascular diseases were analyzed,and the pharmacological care points for frequently used traditional Chinese medicines and those with high risks were summarized.RESULTS:A total of 3 741 inpatient TCM prescriptions were collected.The properties were mainly neutral,warm,and the flavors were predominantly sweet and bitter,the meridian tropism was primarily the lung and liver.Among the top 30 most frequently used Chinese medicines,23 exceeded the average dosage specified in the Pharmacopoeia of the People's Republic of China or the Beijing Specifications for the Processing of Traditional Chinese Medicinal Preparations.Association rule analysis revealed that the herb pairs with the highest support and co-occurrence were Carthami flos-Persicae semen and Poria cocos-Codonopsis radix.Cluster analysis revealed four core TCM combinations.(1)Poria cocos,Radix salviae,Codonopsis radix and Hedysarum Multijugum Maxim;(2)Radix bupleuri and Scutellariae radix;(3)Atractylodes macrocephala koidz,Pinelliae rhizoma praeparatum and Citrus reticulata;(4)Angelicae sinensis radix,Chuanxiong rhizoma,Achyranthis bidentatae radix,Paeoniae radix rubra,Persicae semen and Carthami flos.CONCLUSIONS:Strengthening the monitoring of the combined use of Chinese and Western medicines and adverse reactions during clinical medication for cardiovascular diseases can provide key pharmacovigilance monitoring points and reference basis for clinical Chinese medicine pharmacists in cardiology.
