基于伏邪理论的溃疡性结肠炎复发病机认识OA
Understanding the Pathogenesis of Ulcerative Colitis Recurrence Based on the Latent Pathogen Theory
伏邪作为感邪后伏匿体内、延迟发病的致病因素,是溃疡性结肠炎(UC)反复发作的核心病因.伏邪隐匿性、渐进性、积聚性的特征与UC"正气亏虚为本、湿热瘀结为标"的病机契合,并能与现代医学所证实的黏膜屏障损伤、组织驻留记忆细胞持续活化等病理环节相互印证.基于伏邪理论,本文探讨UC复发的病机演变规律及相应治疗原则,并以"脾胃病辨证新八纲"作为缓解期的辨证论治法则,为中医药防治UC复发提供新思路.
Latent pathogens,as pathogenic factors that remain hidden in the body after infection and cause delayed onset,are the core cause of recurrent ulcerative colitis(UC).The characteristics of latency,progression,and accumulation of latent pathogens align with the pathogenesis of UC,which is"deficiency of vital Qi as the root,and damp-heat and blood stasis as the manifestation".This perspective also correlates with pathological mechanisms confirmed by modern medicine,such as mucosal barrier damage and persistent activation of tissue-resident memory cells.Based on the latent pathogen theory,this paper explores the evolutionary patterns of UC recurrence and corresponding treatment principles.It also proposes the"new eight principles of syndrome differentiation for spleen and stomach diseases"as a guideline for syndrome differentiation and treatment during remission,offering new insights into the prevention and treatment of UC recurrence with traditional Chinese medicine.
