基于"络病理论"探讨糖尿病视网膜病变与糖尿病肾病的相关性及防治策略OA
Explore the Correlation between Diabetic Retinopathy and Diabetic Nephropathy as well as Corresponding Prevention and Treatment Strategies based on the Collateral Dis-ease Theory
糖尿病视网膜病变(DR)与糖尿病肾病(DN)作为糖尿病常见微血管并发症,二者具有共同的发病基础.在相同危险因素的驱动下,两者发展趋势相近,且一方病情加重可加速另一方病理进程,进而严重影响患者生存质量.本文立足于中医医学"络病理论",系统探讨DR和DN的内在联系,认为二者均属"络病"范畴,为消渴日久耗伤气血,致络气郁滞,络脉瘀阻,进而累及眼络、肾络,导致相应病变.同时基于"络病理论"的核心思想提出临床防治的整体观念,以"通络"为核心,遵循"早期固本轻宣,中期攻补兼施,晚期慎通护络"的原则,同时结合现代医学血糖调控、血管保护等干预手段,制定个体化防治方案,实现中西医协同增效,旨在为糖尿病微血管并发症的防治提供新思路.
Studies have demonstrated that diabetic retinopathy(DR)and diabetic nephropathy(DN)possess a common pathogenic basis.Driven by the same etiological risk factors,DR and DN exhibit comparable developmental trends.The deterioration of one can accelerate the pathological progression of the other,and this condition can impair patients'quality.This study explores the intrinsic association between DR and DN by applying the collateral disease theory.Long-term diabetes induces Qi and blood damage,leading to stagnation of collaterals,thereby involving the collaterals in the eyes and kidneys.Meanwhile,this study is based on the collateral disease theory.It proposes to adhere to the holistic concept as the treatment method,and also integrate modern medical measures to formulate individualized prevention and treatment plants.The purpose is to provide new ideas for the prevention and treatment of diabetic microvascular complications.
