磁共振STIR序列参数优化对急性腰椎骨折骨髓水肿诊断准确率与保守治疗并发症的预测价值OA
Predictive value of optimization of MR STIR sequence parameter for diagnostic accuracy of bone marrow edema of acute lumbar fractures and for complications of conservative treatment
目的:探讨磁共振短时反转恢复(STIR)序列参数优化对急性腰椎骨折骨髓水肿诊断效能的影响,并评估其对保守治疗并发症的预测价值.方法:前瞻性纳入2022年1月至2024年5月吉林市化工医院收治的100例急性腰椎骨折患者,按照随机数表法将其分为常规参数组[反转时间(TI)为180 ms]和优化参数组(TI为210 ms),每组50例.所有患者均采用磁共振系统采集影像,比较两组诊断符合率、水肿体积以及随访情况.采用logistic回归分析并发症发生的危险因素.结果:优化参数组诊断符合率为94.00%,常规参数组为86.00%,两组比较差异无统计学意义(P>0.05).优化参数组诊断灵敏度为93.33%,特异度为95.65%,均较常规参数组的84.44%和88.89%有显著提升,两组比较差异有统计学意义(x2=2.103、2.314,P<0.05).两组治疗后并发症总发生率均降低,差异有统计学意义(x2=8.012,P<0.05),其中邻近节段退变显著减少.logistic回归分析显示,水肿体积每增加1 cm3并发症风险升高139%(OR=2.39,95%CI:1.58~3.61),骨密度T值降低1个标准差风险降低64%(OR=0.36,95%CI:0.19~0.71).年龄对并发症无影响,差异无统计学意义(P>0.05).结论:STIR序列TI值优化至210 ms可显著提高骨髓水肿诊断准确率,降低保守治疗并发症风险,水肿体积与骨密度构成是并发症发生的危险因素.
Objective:To investigate the impact of optimization of sequence parameters of Short-Tau Inversion Recovery(STIR)of magnetic resonance(MR)on diagnostic efficacy for bone marrow edema of acute lumbar fractures,and assess the predictive value of that for complications of conservative treatment.Methods:A prospective cohort of 100 patients with acute lumbar fractures,who admitted to hospital between January 2022 and May 2024,was enrolled.Participants were randomly divided into conventional parameter group(inversion time,TI=180 ms)and optimized parameter group(TI=210 ms)according to random number table.All subjects underwent magnetic resonance imaging(MRI)to collect images.The consistence rate of diagnosis,edema volume,and follow-up outcomes were compared between the two groups.Logistic regression was employed to analyze risk factors of occurring complication.Results:The consistence rate of diagnosis was 94.00%in the optimized parameter group,and that was 86.00%in the conventional parameter group,and the difference was not statistically significant(P>0.05).The diagnostic sensitivity and specificity of the optimized parameter group were respectively 93.33%and 95.65%,all of which were significantly higher than 84.44%and 88.89%of the conventional parameter group,and the differences of them were significant(x2=2.103,2.314,P<0.05).The total incidences of complications after treatment were decreased in the two groups,and the difference was significant(x2=8.012,P<0.05),and the degeneration of adjacent segment significantly decreased.Logistic regression analysis revealed that the risk of complications increased by 139%when edema volume increased 1 cm3[OR=2.39,95%Confidence Interval(CI):1.58-3.61)].The risk decreased 64%when T value of bone density decreased 1 standard deviation(OR=0.36,95%CI:0.19-0.71).The age of patient did not exhibit a statistically significant influence on complication(P>0.05).Conclusion:Optimized 210 ms of the TI values of STIR sequence can significantly enhance the diagnostic accuracy for bone marrow edema,and can reduce the risk of occurring complications in conservative treatment.Edema volume and composition of bone density are risk factors of occurring complications.
