Effects of Electroacupuncture Stimulation with Different Parameters Combined with Hand Function Training Device on Upper Limb Motor Function in Stroke-Induced Hemiplegia

目的:观察不同参数电针刺激联合手功能训练仪对脑卒中偏瘫上肢运动功能的影响.方法:选取2023年9月—2024年12月永康市第一人民医院收治的脑卒中偏瘫患者94例,根据随机数字表法分为连续波组和疏密波组各47例.2组患者均采用常规康复方案、手功能训练仪及电针刺激治疗,连续波组采用连续波电针刺激,疏密波组采用疏密波电针刺激,2组均治疗6周.评价2组临床疗效,比较2组功能恢复情况、血清指标、关节活动度、上肢运动功能及生活质量.结果:疏密波组总有效率为91.49%(43/47),高于连续波组74.47%(35/47)(P<0.05).治疗后,2组上肢Brunstrom分期、手Brunstrom分期较治疗前均有改善,且疏密波组优于连续波组,差异均有统计学意义(P<0.05).治疗后,2 组白细胞计数(WBC)、C-反应蛋白(CRP)、白细胞介素-6(IL-6)水平较治疗前降低,且疏密波组低于连续波组,差异均有统计学意义(P<0.05).治疗后,2组腕背伸主动关节活动度(AROM)、Fugl-Meyer评定量表上肢部分(FMA-UE)评分、改良Barthel指数(MBI)评分较治疗前升高,且疏密波组高于连续波组,差异均有统计学意义(P<0.05).结论:疏密波电针刺激联合手功能训练仪可以增强脑卒中偏瘫患者上肢运动功能,提高日常生活能力,临床治疗效果明显.

Objective:To observe the effects of electroacupuncture stimulation with different parameters combined with a hand function training device on upper limb motor function in stroke-induced hemiplegia.Methods:A total of 94 stroke-induced hemiplegia patients admitted to Yongkang First People's Hospital between September 2023 and December 2024 were selected and divided into the continuous wave group(n=47)and the dense-disperse wave group(n=47)using a random number table method.Both groups received conventional rehabilitation therapy,hand function training device intervention,and electroacupuncture stimulation.The continuous wave group stimulated with continuous wave electroacupuncture,while the dense-disperse wave group stimulated with dense-disperse wave electroacupuncture.Treatment duration was six weeks for both groups.Clinical efficacy,functional recovery,serum indicators,joint range of motion,upper limb motor function,and quality of life were compared between the two groups.Results:The total effective rate in the dense-disperse wave group was 91.49%(43/47),significantly higher than that of 74.47%(35/47)in the continuous wave group(P<0.05).After treatment,both groups showed improvement in upper limb and hand Brunstrom stages compared to baseline,with the dense-disperse wave group demonstrating superior outcomes to the continuous wave group(P<0.05).Levels of white blood cell count(WBC),C-reactive protein(CRP),and interleukin-6(IL-6)decreased in both groups after treatment,with lower levels in the dense-disperse wave group(P<0.05).Active range of motion(AROM)for wrist dorsiflexion,Fugl-Meyer Assessment of Upper Extremity(FMA-UE)scores,and Modified Barthel Index(MBI)scores increased in both groups after treatment,with higher improvements in the dense-disperse wave group(P<0.05).Conclusion:Dense-disperse wave electroacupuncture stimulation combined with a hand function training device can significantly enhance upper limb motor function,improve daily living ability,and demonstrate superior clinical efficacy in stroke patients with hemiplegia.
