化瘀消痛汤联合刺络放血治疗腰椎间盘突出症临床研究OA
Clinical Study on Huayu Xiaotong Decoction Combined with Collateral Puncture and Bloodletting Therapy for Lumbar Disc Herniation
目的:观察化瘀消痛汤联合刺络放血治疗腰椎间盘突出症(LDH)的临床疗效.方法:选取2021年1月—2024年9月宁波市镇海区中医医院收治的98例LDH患者的临床资料进行回顾性分析,按治疗方法不同分为对照组和研究组各49例.对照组给予常规牵引训练和镇痛药物对症治疗,研究组加化瘀消痛汤联合刺络放血治疗.2组均治疗4周.评价2组临床疗效,比较2组治疗前后Oswestry功能障碍指数(ODI)评分、疼痛视觉模拟评分法(VAS)评分、血清疼痛介质[前列腺素E2(PGE2)、缓激肽(BK)、5-羟色胺(5-HT)]水平及血液流变学指标[纤维蛋白原(FIB)、全血高切黏度(WBHSV)、全血低切黏度(WBLSV)、血浆黏度(PV)].结果:研究组总有效率为93.88%(46/49),高于对照组77.55%(38/49)(P＜0.05).治疗后,2组ODI及VAS评分较治疗前降低(P＜0.05),且研究组ODI及VAS评分低于对照组(P＜0.05).治疗后,2组PGE2、BK、5-HT水平较治疗前降低(P＜0.05),且研究组PGE2、BK、5-HT水平低于对照组(P＜0.05).治疗后,2组FIB、WBHSV、WBLSV、PV水平较治疗前降低(P＜0.05),且研究组FIB、WBHSV、WBLSV、PV水平低于对照组(P＜0.05).结论:化瘀消痛汤联合刺络放血治疗LDH的疗效显著,可有效减轻患者的疼痛情况,缓解腰椎功能障碍,促进血流动力学的恢复.
Objective:To observe the clinical effect of Huayu Xiaotong Decoction combined with collateral puncture and bloodletting therapy in the treatment of lumbar disc herniation(LDH).Methods:A retrospective analysis was conducted on the clinical data of 98 LDH patients admitted to Zhenhai Hospital of Traditional Chinese Medicine from January 2021 to September 2024.They were divided into the control group and the study group according to different treatment methods,with 49 cases in each group.The control group received conventional traction training and symptomatic treatment with analgesic medications,while the study group was additionally treated with Huayu Xiaotong Decoction combined with collateral puncture and bloodletting therapy.Both groups were treated for four weeks.The clinical effects in the two groups were evaluated,and the Oswestry Disability Index(ODI)scores,Visual Analogue Scale(VAS)scores,serum pain mediator[prostaglandin E2(PGE2),bradykinin(BK),5-hydroxytryptamine(5-HT)]levels and hemorheological indicators[fibrinogen(FIB),whole blood high shear viscosity(WBHSV),whole blood low shear viscosity(WBLSV),plasma viscosity(PV)]were compared between the two groups before and after treatment.Results:The total effective rate in the study group was 93.88%(46/49),which was higher than 77.55%(38/49)in the control group(P＜0.05).After treatment,the ODI and VAS scores of both groups were decreased when compared with those before treatment(P＜0.05),and the ODI and VAS scores in the study group were lower than those in the control group(P＜0.05).After treatment,the levels of PGE2,BK,and 5-HT in both groups were decreased when compared with those before treatment(P＜0.05),and the levels of PGE2,BK,and 5-HT in the study group were lower than those in the control group(P＜0.05).After treatment,the levels of FIB,WBHSV,WBLSV,and PV in both groups were decreased when compared with those before treatment(P＜0.05),and the levels of FIB,WBHSV,WBLSV,and PV in the study group were lower than those in the control group(P＜0.05).Conclusion:Huayu Xiaotong Decoction combined with collateral puncture and bloodletting therapy has a significant effect in the treatment of LDH,which can effectively reduce patients'pain,alleviate lumbar dysfunction,and promote the recovery of hemodynamics.
