目的 探究骨水泥污染骨科关节置换术复用器械经超声波清洗后的细菌残留情况及影响因素.方法 选取2023年2月至2025年1月于江苏省人民医院宿迁医院消毒供应中心进行超声波清洗的骨水泥污染的骨科关节置换术复用器械944件为研究对象,统计其超声波清洗后的细菌残留情况,对可能导致细菌残留超标的因素进行单因素和多因素Logistic分析.结果 在经超声波清洗的944件骨水泥污染骨科关节置换术复用器械中,有92件(9.75%)检出细菌残留超标.单因素和多因素Logistic回归分析结果显示,手术器械为复杂结构、乙醇预处理时间≤30 min、器械被骨水泥严重污染、未使用适宜水质及清洗时未加用碱性洗剂是导致细菌残留超标的独立危险因素(OR>1,P<0.05),而手工清洗+超声波清洗为避免细菌残留超标检出的保护性因素(OR<1,P<0.05).结论 骨水泥污染骨科关节置换术复用器械经超声波清洗后细菌残留超标率较高,复杂器械结构、乙醇预处理时间较短及被骨水泥严重污染的器械更容易发生细菌残留超标,而使用适宜水质、清洗时加用碱性洗剂及手工清洗+超声波清洗有利于降低细菌残留超标的发生率.

Objective To explore the bacterial residue situation and influencing factors of bone cement-contaminated reusable instruments for orthopedic joint replacement surgery after ultrasonic cleaning.Methods Among 944 bone cement-contaminated reusable instruments for orthopedic joint replacement surgery that were ultrasonically cleaned in the disinfection and supply center of Suqian Hospital of Jiangsu Provincial People's Hospital from February 2023 to January 2025 were selected as the research objects.The bacterial residue situation after ultrasonic cleaning was counted,and univariate and multivariate Logistic analysis were conducted on the factors that might lead to excessive bacterial residue.Results Among the 944 bone cement-contaminated reusable instruments for orthopedic joint replacement surgery after ultrasonic cleaning,92 pieces(9.75%)were detected with excessive bacterial residue.The results of univariate and multivariate Logistic regression analysis showed that complex instrument structure,ethanol pretreatment time≤30 minutes,severe contamination of instruments with bone cement,failure to use appropriate water quality,and failure to add an alkaline cleaning agent were the independent risk factors for excessive bacterial residue(OR>1,P<0.05),while manual cleaning+ultrasonic cleaning was a protective factor to avoid the detection of excessive bacterial residue(OR<1,P<0.05).Conclusion The rate of excessive bacterial residue of bone cement-contaminated reusable instruments for orthopedic joint replacement surgery after ultrasonic cleaning is relatively high.Instruments with a complex structure,short ethanol pre-treatment time,and severe bone cement contamination are more likely to have excessive bacterial residue.In contrast,the use of appropriate water quality,addition of alkaline cleaning agent during cleaning,and manual cleaning+ultrasonic cleaning are conducive to reducing the incidence of excessive bacterial residue.
