达芬奇机器人手术器械清洗质量及其影响因素研究OA
Study on cleaning quality of da Vinci robotic surgical instruments and its influencing factors
目的 评估达芬奇机器人手术器械的清洗质量,分析影响其清洗质量的因素.方法 采用横断面调查方法,对西北地区16家三甲医院的达芬奇机器人手术器械清洗现状进行评估.通过自行设计的清洗质量影响因素检查表和清洗现状调查问卷收集数据,并对916件达芬奇机器人手术器械进行随机抽样检测.采用单因素和多因素Logistic回归分析手术器械清洗质量的影响因素.结果 调查显示,各医院的消毒供应中心是主要的达芬奇机器人手术器械管理机构,护士在达芬奇机器人手术器械清洗操作中仍占有较高比例,有25%的医院仅使用视觉检查法来检查清洁效果.单因素分析显示,设备分类清洗(χ²=3.396,P=0.012)、正确的清洗程序(χ²=236.148,P<0.001)、清洗方法(χ²=126.320,P=0.001)、合适的清洗工具(χ²=1.630,P=0.036)、预处理(χ²=8.322,P=0.003)以及清洗人员具备足够的手术器械清洗知识(χ²=4.211,P=0.004)均与清洗质量显著相关(P<0.05).多元Logistic回归分析显示,正确的清洗程序(β=-1.596,P<0.001)、清洗人员具备足够的手术器械清洗知识(β=-0.709,P<0.001)、超声波清洗(β=1.469,P<0.001)和预处理(β=0.463,P<0.001)与清洗质量显著相关.结论 提升达芬奇机器人手术器械清洗质量需规范清洗流程、个性化清洗方法、加强预处理、提升清洗人员专业素养,以提高手术器械清洗质量.
Objective To evaluate the cleaning quality of da Vinci robotic surgical instruments and analyze the factors affecting its cleaning quality.Methods A cross-sectional survey method was adopted to evaluate the current status of da Vinci robotic surgical instrument cleaning in 16 tertiary hospitals in Northwest Region.Data were collected through a self-designed checklist of factors influencing cleaning quality and a questionnaire on the current cleaning status,and random sampling tests were conducted on 916 da Vinci robotic surgical instruments.Univariate and multivariate Logistic regression analysis were used to examine the factors influencing cleaning quality of surgical instruments.Results The survey showed that the disinfection supply center of each hospital was the main management agency for da Vinci robotic surgical instruments,and nurses still account for a high proportion in the cleaning operation of da Vinci robotic surgical instruments,25% of hospitals only use visual inspection to check the cleaning effect.Univariate analysis showed that equipment classification cleaning(χ²=3.396,P=0.012),correct cleaning procedures(χ²=236.148,P<0.001),cleaning methods(χ²=126.320,P=0.001),appropriate cleaning tools(χ²=1.630,P=0.036),pretreatment(χ²=8.322,P=0.003),and cleaning staff having sufficient knowledge of surgical instrument cleaning(χ²=4.211,P=0.004)were all significantly associated with cleaning quality.Multivariate Logistic regression analysis showed that the correct cleaning procedures(β=-1.596,P<0.001)and cleaning staff having sufficient knowledge of surgical instrument cleaning(β=-0.709,P<0.001),ultrasonic cleaning(β=1.469,P<0.001)and pretreatment(β=0.463,P<0.001)were significantly associated with cleaning quality.Conclusion To improve the cleaning quality of da Vinci robotic surgical instruments,it is necessary to standardize the cleaning process,personalize the cleaning method,strengthen pretreatment,improve the professional quality of cleaning staff,so as to improve the cleaning quality of surgical instruments.
