基于"土壅木郁"理论探讨Sestrin2通路防治肥胖2型糖尿病OA
Exploring the Sestrin2 Pathway Against Obese T2DM Based on the Theory of"Tu Yong Mu Yu"
基于"土壅木郁"理论,探讨应激诱导蛋白2(Stress-induced protein 2,Sestrin2)、Kelch样ECH关联蛋白1(Kelch-like ECH associated protein 1,Keap1)和核因子红细胞系2相关因子(Nuclearfactor erythroidderived 2-like2,Nrf2)信号通路在肥胖2型糖尿病(Type 2 diabetes mellitus,T2DM)中的作用机制,内质网应激(Endoplasmic reticulum stress,ERS)是连接肥胖与T2DM 的关键病理环节,而Sestrin2/Keap1/Nrf2通路通过激活抗氧化应激、改善 ERS和糖脂代谢紊乱,重塑胰岛β细胞功能,改善胰岛素抵抗(Insulin resistance,IR)."土壅木郁"理论与ERS的现代医学机制高度契合,健脾祛湿,疏肝降浊法可能通过上调Sestrin2表达,抑制 Keap1活性,激活 Nrf2核转录因子,减轻 ERS诱导的IR,延缓T2DM进展,为中医药通过调节 Sestrin2/Keap1/Nrf2通路改善肥胖T2DM 提供理论依据.
Based on Tu Yong Mu Yu theory to explore the mechanism of action of the stress-induced protein 2(Sestrin2),Kelch-like ECH-associated protein 1(Keap1),and nuclear factor erythroid 2-related factor 2(Nrf2)signa-ling pathways in obese type 2 diabetes mellitus(T2DM).Endoplasmic reticulum stress(ERS)is a key pathological link connecting obesity and T2DM,while the Sestrin2/Keap1/Nrf2(reticulum stress,ERS)is a key pathological link connecting obesity,and T2DM.The Sestrin2/Keap1/Nrf2 pathway activates antioxidant stress,improves ERS and lipid metabolism disorders,restores pancreatic β-cell function,and alleviates insulin resistance(IR).The"Tu Yong Mu Yu"theory aligns closely with the modern medical mechanisms of ERS.Methods such as strengthening the spleen and re-moving dampness,and regulating the liver and clearing turbidity may upregulate Sestrin2 expression,inhibit Keap1 ac-tivity,activate the Nrf2 nuclear transcription factor,alleviate ERS-induced IR,and delay the progression of T2DM.This study provides a theoretical basis for traditional Chinese medicine to regulate the Sestrin2/Keap1/Nrf2 pathway to im-prove obese T2DM.
