溃结宁膏穴位贴敷对脾肾阳虚型溃疡性结肠炎大鼠的治疗作用及机制研究OA
Therapeutic Effect and Mechanism of Kuijiening Ointment Acupoint Application on Rats with Ulcerative Colitis of Spleen-Kidney Yang Deficiency Syndrome
目的:观察溃结宁膏穴位贴敷对脾肾阳虚型溃疡性结肠炎(UC)大鼠模型的治疗效果,并探讨其对肠道炎症、屏障功能及肠道菌群的调节作用.方法:从60只雄性SPF级SD大鼠中随机选取50只,采用腺嘌呤与番泻叶联合灌胃结合寒湿环境刺激法建立脾肾阳虚证候模型,并以三硝基苯磺酸(TNBS)灌肠诱导UC,构建脾肾阳虚型UC复合模型.将成模大鼠随机分为模型组、穴位贴敷组、非穴位贴敷组、穴位非贴敷组和柳氮磺吡啶(SASP)组,剩余未造模大鼠作为空白组.对所有组大鼠均连续干预14 d,其中空白组与模型组予以生理盐水灌胃,穴位贴敷组在特定穴位贴敷溃结宁膏并辅以生理盐水灌胃,非穴位贴敷组在臀部非穴位区贴敷溃结宁膏并辅以生理盐水灌胃,穴位非贴敷组在特定穴位贴敷空白巴布剂并辅以生理盐水灌胃,SASP组则予以SASP灌胃.连续干预后,评估各组大鼠疾病活动指数(DAI)、结肠组织损伤指数(TDI)、结肠组织病理变化,检测结肠组织中肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-4)水平及紧密连接蛋白(ZO-1、Occludin)表达,通过 16S rRNA技术分析肠道菌群多样性及优势菌门丰度变化,并进行对比分析.结果:与空白组比较,模型组大鼠结肠长度明显缩短,DAI、TDI及结肠组织中TNF-α水平均显著升高(P均<0.01),体质量及IL-4水平显著降低(P<0.01),结肠组织中ZO-1、Occludin蛋白表达均明显降低(P<0.05、P<0.01),肠道菌群多样性明显降低,厚壁菌门(Firmicutes)菌群相对丰度降低(P<0.05),拟杆菌门(Bacteroidetes)菌群相对丰度升高(P<0.01).穴位贴敷组与SASP组大鼠结肠长度均明显长于模型组,DAI、TDI及结肠组织中TNF-α水平均显著低于模型组(P均<0.01),体质量、结肠组织中IL-4水平及Occludin蛋白明显升高(P<0.01),ZO-1蛋白表达亦有所升高(P<0.05 或P<0.01),肠道菌群多样性改善,厚壁菌门相对丰度升高(P<0.05、P<0.01),拟杆菌门相对丰度降低(P<0.01).结论:溃结宁膏穴位贴敷可能通过抑制炎症反应、修复肠黏膜屏障、调节肠道菌群平衡等多途径,发挥对脾肾阳虚型UC的治疗作用.
Objective To observe the therapeutic effect of Kuijiening Ointment acupoint application on rat models with ulcerative colitis(UC)of spleen-kidney yang deficiency syndrome,and to explore its regulatory effects on intestinal inflammation,intestinal barrier function and intestinal flora.Methods Total 50 rats were randomly selected from 60 male SPF-grade SD rats.Rat models of spleen-kidney yang deficiency syn-drome were established by intragastric administration of adenine and Fanxieye(Sennae Folium)combined with cold-damp environment stimulation,and UC was induced by 2,4,6-trinitrobenzene sulfonic acid(TNBS)enema to construct a composite model with UC of spleen-kidney yang deficiency syndrome.The successfully modeled rats were randomly divided into 5 groups:the model group,acupoint application group,non-acupoint application group,acupoint placebo application group and sulfasalazine(SASP)group,with the remaining non-modeled rats serving as the blank group.All groups were intervened continuously for 14 days.The blank group and model group were given normal saline by gavage.The acupoint application group received Kuijiening Oint-ment application at specific acupoints and normal saline gavage.The non-acupoint application group was treated with Kuijiening Ointment applied to the non-acupoint area of the buttocks and normal saline gavage.The acu-point placebo application group was given blank cataplasm application at specific acupoints and normal saline gavage.The SASP group was administered SASP by gavage.After continuous intervention,the disease activ-ity index(DAI)and colonic tissue damage index(TDI)of rats in each group were evaluated,and the pathologi-cal changes of colonic tissues were observed.The levels of tumor necrosis factor-α(TNF-α)and interleukin-4(IL-4)in colonic tissues,as well as the expression of tight junction proteins(ZO-1,Occludin)were detected.The diversity of intestinal flora and the changes in the abundance of dominant phyla were analyzed by 16S rRNA gene sequencing technology,followed by comparative analysis.Results Compared with the blank group,the colonic length of rats in the model group was significantly shortened,the levels of DAI,TDI and TNF-α in colonic tissues were significantly increased(all P<0.01),the body weight and IL-4 level were significantly decreased(P<0.01),the expression of ZO-1 and Occludin proteins in colonic tissues was significantly reduced(P<0.05,P<0.01),the diversity of intestinal flora was markedly decreased,the relative abundance of Fir-micutes was reduced(P<0.05),and the relative abundance of Bacteroidetes was increased(P<0.01).Com-pared with the model group,the colonic length of rats in the acupoint application group and SASP group was significantly longer,the levels of DAI,TDI and TNF-α in colonic tissues were significantly lower(all P<0.01),the body weight,IL-4 level and Occludin protein expression in colonic tissues were significantly increased(P<0.01),the expression of ZO-1 protein was also elevated(P<0.05 or P<0.01),the diversity of intestinal flora was improved,the relative abundance of Firmicutes was increased(P<0.05,P<0.01),and the relative abundance of Bacteroidetes was decreased(P<0.01).Conclusion Acupoint application of Kui-jiening Ointment may exert the therapeutic effect on UC of spleen-kidney yang deficiency syndrome through multiple pathways,including inhibiting inflammatory response,repairing intestinal mucosal barrier and regulat-ing the balance of intestinal flora.
