目的:分析不同中医证型青壮年近视患者视神经纤维层厚度和黄斑区视网膜厚度的差异,探讨中医证型与近视眼视神经纤维层厚度和黄斑区视网膜厚度变化之间的关联.方法:对临床268例(268眼)青壮年近视患者进行中医辨证分型,比较不同中医证型间视神经纤维层厚度和黄斑区视网膜厚度的差异,并分析其相关因素.结果:①中医辨证分型:268例近视患者中:心阳不足66例(24.63%),气血不足102例(38.06%),肝肾两虚100例(37.31%).三组证型在性别和年龄上的差异无统计学意义(P>0.05);在屈光度和眼轴长度上的差异具有统计学意义(P<0.001),屈光度呈降低趋势,眼轴长度呈升高趋势.②视神经纤维层厚度的比较:三组证型间差异具有统计学意义(P<0.05).除颞侧外,视神经纤维层厚度呈变薄趋势(心阳不足>气血不足>肝肾两虚).③黄斑区视网膜厚度的比较:在中央区,三组证型的差异无统计学意义(P>0.05);在内环区和外环区,除外环颞侧外,三组证型间的差异均具有统计学意义(P<0.05),视网膜厚度呈变薄趋势(心阳不足>气血不足>肝肾两虚).④视神经纤维层厚度的相关性分析:与性别无相关(P>0.05),与年龄负相关(P<0.05),除颞侧外,与屈光度正相关(P<0.05),与眼轴长度负相关(P<0.05).⑤黄斑区视网膜厚度的相关性分析:与性别负相关(P<0.05),与年龄无相关(P>0.05);在中心凹区:视网膜厚度与屈光度无相关(P>0.05),与眼轴长度正相关(P<0.05);在其他区域:视网膜厚度与屈光度正相关(P<0.05),与眼轴长度负相关(P<0.05).结论:青壮年近视患者证型以气血不足和肝肾两虚为多见.不同中医证型间的屈光度、眼轴长度、视神经纤维层厚度、黄斑区视网膜厚度均存在一定差异,肝肾两虚型屈光度更高、眼轴更长、视神经纤维层厚度和黄斑区视网膜厚度更薄.视神经纤维层厚度和黄斑区视网膜厚度的变化与屈光度、眼轴长度密切相关,与性别、年龄部分相关.该研究结果可为中医辨证施治提供参考依据.

Objective:To analyze the differences of retinal nerve fiber layer and macula retinal thickness in young myopic patients with different traditional Chinese medicine(TCM)syndrome types,and to explore the correlation between TCM syndrome types with retinal nerve fiber layer and macula retinal thickness of myopia.Methods:In this study,268 cases(268 eyes)of young myopic patients were classified by TCM syndrome differentiation.The differences of retinal nerve fiber layer thickness and macular retinal thickness between different TCM syndrome types were compared and the related factors were analyzed.Results:①TCM syndrome differentiation:Among 268 myopic patients,66 cases(24.63%)of deficiency of heart Yang,102 cases(38.06%)of deficiency of Qi and blood,100cases(37.31%)of defi-ciency of liver and kidney.There were no statistical significance in gender and age of the three groups of syndrome types(P>0.05);but there were statistically significant differences in diopter and axial length(P<0.001),the diopter showed a decreasing trend,while axial length showed a increasing trend.②Comparison of retinal nerve fiber layer thickness:The difference between the three groups of syndrome types was statistically significant(P<0.05).Except for the temporal area,the retinal nerve fiber layer thickness became thinner(deficiency of heart Yang>deficiency of Qi and blood>deficiency of liver and kidney).③Comparison of macular retinal thickness:In central area,there was no significant difference in the three groups of syndrome types(P>0.05);In intra-macular ring areas and extra-macular ring areas,except for the extra temporal area,the differences of the syndrome types in the three groups were statistical-ly significant(P<0.05),and macular retinal thickness became thinner(deficiency of heart Yang>deficiency of Qi and blood>deficiency of liver and kidney).④Correlation analysis of retinal nerve fiber layer thickness:No correlation with gender(P>0.05),negative correlation with age(P<0.05).Positive correlation with diopter(P<0.05),nega-tive correlation with axial length(P<0.05),except for the temporal area.⑤Correlation analysis of macular retinal thickness:negative correlation with gender(P<0.05);no correlation with age(P>0.05);In fovea:retinal thickness was not correlated with diopter(P>0.05),but was positive correlated with axial length(P<0.05);In other areas:retinal thickness was positive correlated with diopter(P<0.05)and negative correlated with axial length(P<0.05).Conclusion:Deficiency of Qi and blood and deficiency of liver and kidney are the most common syndromes in young myopic patients.The diopter,axial length,retinal nerve fiber layer thickness,macular retinal thickness have some differences among different TCM syndromes.The diopter was higher,the axial length was longer,the thickness of reti-nal nerve fiber layer and macular retinal were thinner in deficiency of liver and kidney patients.The changes of thick-ness of retinal nerve fiber layer and macula are closely related to the diopter and axial length,and partly related to gen-der and age.The findings will provide reference for TCM syndrome differentiation and treatment of young myopic pa-tients.
