多发性骨髓瘤组织circ-PTK2和circ-ITCH表达及其与患者临床病理特征和预后的关系OA
Expressions of circ-PTK2 and circ-ITCH in multiple myeloma tissues and their relationship with patients'clinicopathological characteristics and prognosis
目的 探究多发性骨髓瘤(multiple myeloma,MM)组织中环状RNA蛋白酪氨酸激酶2(circular RNA protein tyrosine kinase 2,circ-PTK2)和环状RNA泛素E3 连接酶(circular RNA itchy E3 ubiquitin protein ligase,circ-ITCH)的表达及其与患者临床病理特征和预后的关系.方法 选择 2017 年 1 月至 2020 年 8 月本院收治的MM患者 93 例作为研究对象(MM组),另选取同期本院骨髓象检查正常者 93 例作为对照组,通过qPCR检测组织中circ-PTK2 和circ-ITCH的表达.根据 3 年内随访结果,将患者分为死亡组(n=42)和生存组(n=51).采用Kaplan-Meier法分析circ-PTK2 和circ-ITCH表达与MM患者预后的关系.采用多因素logistic回归分析影响MM患者预后的因素.受试者工作特征(receiver operating characteristic,ROC)曲线评估circ-PTK2 和circ-ITCH表达预测MM患者预后不良的价值.结果 MM组织中circ-PTK2 表达较正常骨髓组织升高,circ-ITCH表达则降低(均P<0.05).circ-PTK2 和circ-ITCH表达在国际分期系统(International Staging System,ISS)分期、白蛋白(albumin,Alb)水平和血肌酐(serum creatinine,Scr)水平方面比较,差异均具有统计学意义(均P<0.05).Kaplan-Meier生存曲线显示,采用表达水平的平均值分组,circ-PTK2 高表达患者 3 年总生存率低于低表达者,circ-ITCH低表达患者 3 年总生存率低于高表达者(均P<0.05).死亡组和生存组在ISS分期、血清Alb与Scr水平和circ-PTK2 与circ-ITCH表达方面比较,差异均具有统计学意义(均P<0.05).多因素logistic分析显示,ISS分期、Scr水平和circ-PTK2 高表达为影响MM患者预后的危险因素,circ-ITCH高表达为保护因素(均P<0.05).circ-PTK2 和circ-ITCH表达联合预测MM患者预后的ROC曲线下面积(area under the curve,AUC)为0.878,大于circ-PTK2和circ-ITCH单独预测的AUC(均P<0.05).结论 MM组织中circ-PTK2高表达,circ-ITCH低表达,与患者临床病理特征和预后有关.
Objective To investigate the expressions of circular RNA protein tyrosine kinase 2(circ-PTK2)and circular RNA itchy E3 ubiquitin protein ligase(circ-ITCH)in multiple myeloma(MM)tissues,and their relationship with the clinicopathological characteristics and prognosis of MM patients.Methods A total of 93 MM patients admitted to our hospital from January 2017 to August 2020 were select-ed as the MM group,and 93 normal bone marrow specimen from our hospital collected during the same period were selected as the control group.qPCR was used to detect the expressions of circ-PTK2 and circ-ITCH in tissues.Based on the follow-up results within three years,the patients were divided into the death group(n=42)and survival group(n=51).Kaplan-Meier method was applied to analyze the relation-ship between the expressions of circ-PTK2 and circ-ITCH and the prognosis of MM patients.Multivariate logistic regression was applied to analyze the factors affecting the prognosis of MM patients.Receiver operating characteristic(ROC)curve was used to evaluate the value of circ-PTK2 and circ-ITCH expressions in predicting the poor prognosis of MM patients.Results Compared with the control group,the expression of circ-PTK2 was higher in the MM group,while the expression of circ-ITCH was lower(both P<0.05).The expressions of circ-PTK2 and circ-ITCH were associated with International Staging System(ISS)staging and serum creatinine(Scr)and albumin(Alb)levels(all P<0.05).Kaplan-Meier survival curves showed that grouped by the mean expression level,the 3-year overall survival(OS)rate of patients with high circ-PTK2 expression was lower than that of patients with low circ-PTK2 expression,and the 3-year OS rate of patients with low expression of circ-ITCH was lower than that of patients with high expression of circ-ITCH(both P<0.05).There were significant differ-ences in ISS stage,the levels of serum Alb and Scr,and the expressions of circ-PTK2 and circ-ITCH between the death group and survival group(all P<0.05).Logistic analysis showed that ISS staging,serum Scr level,and high circ-PTK2 expression were risk factors affecting the prognosis of MM patients,while high circ-ITCH expression was a protective factor(all P<0.05).The ROC AUC of the combination of circ-PTK2 and circ-ITCH expressions on predicting the prognosis of MM patients was 0.878,which was larger than that of circ-PTK2 or circ-ITCH expression alone(both P<0.05).Conclusions Circ-PTK2 is highly expressed in MM tissues,while circ-ITCH is lowly expressed,which is related to the clinicopathological characteristics and prognosis of patients.
