目的 探讨不孕女性患者红细胞叶酸水平对体外受精-胚胎移植(IVF-ET)结局的影响,及其在辅助生殖周期中对卵泡募集、卵子成熟、卵子受精与早期胚胎发育的潜在作用.方法 本研究回顾性纳入2021年1-12月于空军军医大学西京医院妇产科生殖中心行IVF-ET治疗的105例不孕女性患者,检测患者取卵前2天红细胞叶酸水平,并比较获卵数、成熟卵数、双原核(2PN)数、2PN卵裂胚胎数和可移植胚胎数,采用线性回归分析红细胞叶酸水平与IVF-ET结局指标的相关性,并按红细胞叶酸水平分成≥800 nmol/L组和<800 nmol/L组,采用独立样本t检验分析两组间IVF-ET各结局指标差异,进一步分析红细胞叶酸水平与IVF-ET各结局指标之间的效应关系.结果 线性回归分析显示,红细胞叶酸水平与获卵数、成熟卵数(MⅡ)数及可移植胚胎数无显著相关性(P>0.05),但与2PN数和2PN卵裂胚胎数呈边缘负相关(P接近0.05).进一步分组后结果显示红细胞叶酸≥800 nmol/L组2PN数、2PN卵裂胚胎数及可移植胚胎数显著低于<800 nmol/L组(P<0.05).结论 体内红细胞叶酸累积到一定的阈值时,可能对卵子受精及胚胎早期发育产生不利影响,故辅助生殖人群叶酸补充应结合个体水平和代谢状态进行个体化干预.

Objective To explore the effect of red blood cell(RBC)folate levels on the outcomes of in vitro fertilization-embryo transfer(IVF-ET)in infertile women,and evaluate its potential role in follicular recruitment,oocyte maturation,oocyte fertilization,and early embryo development during assisted reproductive cycles.Methods A total of 105 infertile women who underwent IVF-ET treat-ment from January to December 2021 in the Reproductive Center of the Department of Obstetrics and Gynecology,Xijing Hospital,Air Force Medical University were enrolled in this retrospective study.RBC folate level was measured two days before oocyte retrieval.The outcomes were compared in terms of the number of retrieved oocytes,mature oocytes,two pronuclei(2PN)oocytes,2PN cleavage embryos,and transferable embryos.Linear regression analysis was used to examine the correlation between RBC folate levels and IVF-ET outcome indicators.The subjects were further divided into≥800 nmol/L group and<800 nmol/L group according to red blood cell folate levels.Independent sample t-test was used to compare the outcome indicators of IVF-ET between the two groups,and the relationship between RBC folate level and IVF-ET outcomes was further analyzed.Results Linear regression analysis showed no significant correlation between RBC folate level and the numbers of retrieved oocytes,mature oocytes,and transferable embryos(P>0.05),but a marginal negative correlation between RBC folate level and the numbers of 2PN oocytes and 2PN cleavage embryos(P value close to 0.050).Further analysis after grouping revealed that the number of 2PN oocytes,2PN cleavage embryos,and transferable embryos were significantly lower in RBC folate≥800 nmol/L group than in<800 nmol/L group(P<0.05).Conclusion When RBC folate accumulates to a certain threshold in the body,it may have an adverse effect on oocyte fertilization and early embryo development.Therefore,folic acid supplementation in assisted reproductive populations should be individualized based on individual levels and metabolic status.
