目的 研究参乌冠心颗粒通过调控NR3C1/GRP78抑制心肌细胞内质网应激缓解冠心病心绞痛的机制.方法 取60只健康雄性SD大鼠,将其均分为正常组、模型组、参乌冠心颗粒低剂量组、参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组、复方丹参滴丸组6组.使用戊巴比妥钠对各组大鼠进行腹腔麻醉,模型组注入垂体后叶素6 U/kg(浓度为2 U/mL),建立冠心病心绞痛模型;正常组注射等量0.9%氯化钠溶液.通过超声心动图检测大鼠心脏功能,所有大鼠均进行心肌组织病理学检测;检测各组大鼠心肌组织中Caspase-12 的活性,RT-PCR、Western blot分别检测大鼠心肌组织中NR3C1、GRP78、CHOP、ATF6 的mRNA和蛋白表达水平.结果 各组大鼠左室射血分数(LVEF)比较:与正常组比较,模型组、参乌冠心颗粒低剂量组、参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组、复方丹参滴丸组均显著降低(均P<0.05);与模型组比较,参乌冠心颗粒低剂量组、参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组、复方丹参滴丸组均显著增加(均P<0.05),其中参乌冠心颗粒低剂量组、参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组随参乌冠心颗粒的给药剂量增加,呈现增长趋势;与参乌冠心颗粒低剂量组比较,参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组、复方丹参滴丸组均显著增加(均P<0.05);与参乌冠心颗粒中剂量组比较,参乌冠心颗粒高剂量组、复方丹参滴丸组均显著增加(均P<0.05);参乌冠心颗粒高剂量组与复方丹参滴丸组比较,二者的治疗效果相当.HE染色结果可见:正常组大鼠心肌组织排列整齐,染色均匀,无炎性浸润;模型组大鼠心肌细胞肥大,胞质染色不均匀,细胞间隙变宽,炎性浸润明显;参乌冠心颗粒低剂量组、参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组、复方丹参滴丸组均较模型组的炎性损伤程度减轻,其中参乌冠心颗粒中剂量组的炎性损伤程度较参乌冠心颗粒低剂量组轻;参乌冠心颗粒高剂量组的炎性损伤程度较参乌冠心颗粒中剂量组轻;复方丹参滴丸组损伤程度与参乌冠心颗粒高剂量组相近.各组大鼠左室舒张末内径(LVEDD),左室收缩末内径(LVESD),Caspase-12活性,NR3C1、GRP78、CHOP、ATF6 的mRNA及蛋白表达水平比较:与正常组比较,模型组、参乌冠心颗粒低剂量组、参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组、复方丹参滴丸组均显著增加(均P<0.05);与模型组比较,参乌冠心颗粒低剂量组、参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组、复方丹参滴丸组均显著减小或降低(均P<0.05),其中参乌冠心颗粒低剂量组、参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组随参乌冠心颗粒的给药剂量增加,呈现降低趋势;与参乌冠心颗粒低剂量组比较,参乌冠心颗粒中剂量组、参乌冠心颗粒高剂量组、复方丹参滴丸组均显著减小或降低(均P<0.05);与参乌冠心颗粒中剂量组比较,参乌冠心颗粒高剂量组、复方丹参滴丸组均显著减小或降低(均P<0.05);参乌冠心颗粒高剂量组与复方丹参滴丸组比较,二者的上述指标水平相当.结论 参乌冠心颗粒可能是通过调控NR3C1和GRP78表达,抑制CHOP、ATF6转录和Caspase-12活性,继而抑制心肌细胞的内质网应激,减轻心肌细胞损伤,从而缓解冠心病心绞痛.

Objective To study the mechanism of Shenwu Guanxin Granules(参乌冠心颗粒)in relieving angina pectoris of coronary heart disease by regulating NR3C1/GRP78 and inhibiting endoplasmic reticulum stress of cardiomyocytes.Methods 60 healthy male SD rats were divided into 6 groups:normal group,model group,low-dose Shenwu Guanxin Granules group,medium-dose Shenwu Guanxin Granules group,high-dose Shenwu Guanxin Granules group,Compound Danshen Dropping Pills(复方丹参滴丸)group.The rats in each group were given peritoneal anesthesia with pentobarbital sodium,and the model group was injected with pituitrin 6 U/kg(concentration 2 U/mL)to establish the angina pectoris model of coronary heart disease.The normal group was injected with the same amount of normal saline.Echocardiography was used to detect the cardiac function of the rats,and myocardial histopathology was performed in all the rats.The activity of Caspase-12 in myocardium of rats in each group was detected.The mRNA and protein expression levels of NR3C1,GRP78,CHOP and ATF6 in myocardium of rats were detected by RT-PCR and Western blot,respectively.Results Comparison of LVEF among groups:Compared with normal group,model group,low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group,all showed significant decreases(all P<0.05);Compared with model group,low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group,all showed significant increases(all P<0.05),among low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,showed an increasing trend as the dosage of Shenwu Guanxin Granules increased;Compared with low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group,all showed significant increases(all P<0.05);Compared with medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group,all showed significant decreases(all P<0.05);The treatment effect of high-dose Shenwu Guanxin Granules group was comparable to that of Compound Danshen Dropping Pills group.HE staining results showed that:in normal group,the myocardial tissue of rats was arranged neatly and stained uniformly,without inflammatory infiltration;in model group,the myocardial cells were hypertrophic,the cytoplasm staining was uneven,the cell gap was widened,and the inflammatory infiltration was obvious;low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group had less inflammatory injury than the model group,among which the inflammatory injury degree of medium-dose Shenwu Guanxin Granules group was less than that of low-dose Shenwu Guanxin Granules group;the inflammatory injury degree of high-dose Shenwu Guanxin Granules group was less than that of medium-dose Shenwu Guanxin Granules group;the injury degree of Compound Danshen Dropping Pills group was similar to that of high-dose Shenwu Guanxin Granules group.Comparison of LVEDD,LVESD,Caspase-12 activity,mRNA and protein expression levels of NR3C1,GRP78,CHOP,and ATF6 among groups:Compared with normal group,model group,low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group,all showed significant increases(all P<0.05);Compared with model group,low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group,all showed significant decreases(all P<0.05),among which low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,showed a decreasing trend as the dosage of Shenwu Guanxin Granules increased;Compared with low-dose,medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group,all showed significant decreases(all P<0.05);Compared with medium-dose and high-dose Shenwu Guanxin Granules groups,Compound Danshen Dropping Pills group,all showed significant decreases(all P<0.05);The above indicators of high-dose Shenwu Guanxin Granules group,Compound Danshen Dropping Pills group were comparable.Conclusion Shenwu Guanxin Granules may regulate the expression of NR3C1 and GRP78,inhibit the transcription of CHOP and ATF6 and the activity of Caspase-12,thereby inhibiting endoplasmic reticulum stress in myocardial cells,reducing myocardial cell injury,and thus alleviating angina pectoris of coronary heart disease.
