中、重型创伤性脑损伤患者不同时间窗压力反应性指数(PRx)对预后的预测价值OA
Prognostic predictive value of pressure reactivity index(PRx)in patients with moderate and severe traumatic brain injury in different time windows
目的 探讨不同时间窗内压力反应性指数(PRx)对中、重度创伤性脑损伤患者预后评估的价值.方法 回顾性分析内蒙古自治区人民医院神经外科 2022 年 11 月—2024 年 1 月神经外科收治的 50 例中、重度创伤性脑损伤患者的临床资料,根据其出院后6 个月的格拉斯哥预后量表(GOS)分为预后良好组27例、预后不良组23 例,使用受试者工作特征(ROC)曲线分析两组患者不同时间段内PRx、颅内压(ICP)及入院格拉斯哥昏迷指数(GCS)等对患者预后的预测价值.结果 两组患者的年龄、性别差异无统计学意义(P>0.05),预后良好组患者入院时GCS大于预后不良组(P<0.05);预后良好组患者术后 0~24h平均PRx、24~48 h平均PRx、48~72 h平均PRx、72~96 h平均PRx、96~120 h平均PRx均小于预后不良组(P<0.001);预后良好组患者平均ICP小于预后不良组(P<0.001);各时间段内的PRx对患者预后均有良好的预测价值(AUC>0.7),48~72 h 时间窗的预测效能最优,其 ROC 曲线下面积为 0.829(95%CI=0.709~0.948),截断值设为0.233 时,灵敏度为69.57%,特异度为92.59%,约登指数0.622,P<0.001.结论 PRx的时间动态特征能够帮助识别最佳的干预时机.各时间窗内的PRx对患者预后均有良好的预测价值,其中48~72 h平均PRx对预后评估价值最高且优于入院GCS和平均ICP.
Objective To explore the value of pressure reactivity index(PRx)in different time periods for the prognostic assessment of patients with moderate and severe traumatic brain injury.Methods The clinical data of 50 patients with moderate and severe traumatic brain injury admitted to the Department of Neurosurgery of the People's Hospital of the Inner Mongolia Autonomous Region from November 2022 to January 2024 were retrospectively analyzed,and according to the Glasgow outcome scale(GOS)of the patients at 6 months after discharge,27 cases of good prognostic group,and 23 cases of poor prognostic group.The predictive value of PRx,intracranial pressure(ICP),and admission Glasgow coma scale(GCS)in different time periods of the patients in the two groups were analyzed by receiver operating characteristic(ROC)curves.Results There was no statistically significant difference in age and gender between the two groups,and the GCS of patients in good prognostic group was higher than that of poor prognostic group at the time of admission(P<0.05).The mean PRx of patients in good prognostic group was smaller than that of poor prognostic group at postoperative periods of 0-24 h,24-48 h,48-72 h,72-96 h and 96-120 h(P<0.001).The mean ICP of patients in the good prognostic group was significantly lower than that in the poor prognostic group(P<0.001).PRx had good predictive value for patient prognosis in all time periods(AUC>0.7),with the best predictive efficacy in the 48-72 h time window,with an area under the ROC curve of 0.829(95%CI=0.709-0.948),and a sensitivity of 69.57%,specificity of 92.59%,and Youden's index of 0.622 with the cut-off value set to 0.233,P<0.001.Conclusions The temporal dynamic characteristics of PRx can help to identify the optimal timing of intervention.PRx within each time window had good predictive value for patient prognosis,with 48-72 h mean PRx having the highest value for prognostic assessment and being superior to admission GCS and mean ICP.
