目的 探讨血清5-羟色胺(5-HT)、CX3C趋化因子配体 1(CX3CL1)对急性脑梗死(ACI)后睡眠障碍的预测价值.方法 采用匹兹堡睡眠质量指数(PSQI)评估93 例ACI患者的睡眠情况,分为睡眠障碍组及对照组.采用ELISA法测定血清 5-HT、CX3CL1 表达水平;采用Spearman秩相关分析探讨血清 5-HT、CX3CL1 水平与PSQI总分的相关性;用Logistic分析研究影响ACI患者睡眠障碍的因素;采用ROC曲线分析血清5-HT、CX3CL1 对 ACI 患者睡眠障碍的预测价值.结果 随访 6 个月时,93 例 ACI 患者中有 34 例(36.56%)发生睡眠障碍.与对照组相比,睡眠障碍组血清 5-HT水平显著降低,CX3CL1 水平显著升高(均P<0.001).ACI患者PSQI总分与5-HT水平呈负相关(rs=-0.526,P<0.05),与CX3CL1 水平呈正相关(rs=0.553,P<0.05).Logistic回归模型分析显示,血清5-HT是ACI睡眠障碍的保护因素(P<0.05),血清CX3CL1 是ACI睡眠障碍的独立危险因素(P<0.05).ROC曲线显示,5-HT、CX3CL1 二者联合预测ACI睡眠障碍优于血清 5-HT、CX3CL1 单独预测(Z=2.563,P=0.010;Z=2.808,P=0.005).结论 血清 5-HT、CX3CL1 与ACI睡眠障碍密切相关,对ACI睡眠障碍具有一定的预测价值.

Objective To explore the predictive value of serum 5-hydroxytryptamine(5-HT)and chemokine C-X3-C ligand 1(CX3CL1)on sleep disorders after acute cerebral infarction(ACI).Methods The Pittsburgh sleep quality index(PSQI)was used to evaluate the sleep status of 93 patients with ACI,and they were divided into sleep disorder group and control group.The levels of serum 5-HT and CX3CL1 were determined by ELISA.Spearman rank correlation analysis was used to analyze the correlation between serum 5-HT,CX3CL1 levels and PSQI total score.Logistic analysis was used to analyze the factors affecting sleep disorders in ACI patients.The ROC curve was used to analyze the predictive value of serum 5-HT and CX3CL1 for sleep disorders in ACI patients.Results At 6 months of follow-up,34 cases(36.56%)of 93 ACI patients had sleep disorders.Compared with those in the control group,the serum 5-HT level was significantly decreased,and the CX3CL1 level was significantly increased in the sleep disorder group(all P<0.001).The total score of PSQI in ACI patients was negatively correlated with the level of 5-HT(rs=-0.526,P<0.05),and positively correlated with the level of CX3CL1(rs=0.553,P<0.05).Logistic regression analysis showed that serum 5-HT was a protective factor for ACI sleep disorder(P<0.05),and serum CX3CL1 was an independent risk factor for ACI sleep disorder(P<0.05).The ROC curve showed that the combined prediction of 5-HT and CX3CL1 was better than that of serum 5-HT and CX3CL1 alone in predicting ACI sleep disorder(Z=2.563,P=0.010;Z=2.808,P=0.005).Conclusion Serum 5-HT and CX3CL1 are closely related to sleep disorders in ACI,and have certain predictive value for sleep disorders in ACI.
