基于"湿热论治、内外并治"的黄白胶囊联合消痔洗剂治疗混合痔术后并发症临床观察OA
Clinical Observation on Huangbai Capsule Combined with Xiaozhi Lotion in the Treatment of Postoperative Complications of Mixed Hemorrhoids Based on the Therapeutic Principles of"Damp-heat Treatment,Internal and External Treatment"
目的:以"湿热论治、内外并治"为指导思想,探究黄白胶囊联合消痔洗剂治疗混合痔术后并发症的临床疗效及对炎症因子的影响.方法:将 90 例混合痔术后患者采用随机数字表法分为黄白胶囊组、消痔洗剂组和联合治疗(黄白胶囊+消痔洗剂)组,各 30 例.记录 3 组患者术后第 3、7、14 天肛门水肿、疼痛、出血情况,完成中医证候积分统计.采用ELISA法检测 3 组NF-κB、IFN-γ表达情况,利用HE染色观察 3 组创面肉芽组织情况.结果:3 组均能有效缓解混合痔术后疼痛、水肿、出血症状及改善中医证候.术后第 14 天比较,联合治疗组改善情况优于消痔洗剂组及黄白胶囊组,差异有统计学意义(P<0.05);3 组愈合时间比较,联合治疗组短于消痔洗剂组及黄白胶囊组,差异有统计学意义(P<0.05);3 组临床总有效率比较无显著性差异(P>0.05).术后第 14 天,3 组NF-κB、IFN-γ表达水平均有下降,且联合治疗组低于其他2 组,差异均有统计学意义(P<0.05).术后第 14 天HE染色光镜下观察发现,联合治疗组成纤维细胞分布相对密集且有序,清晰可见胶原纤维生成及较多毛细血管生成.结论:以"湿热论治、内外并治"理论为指导的黄白胶囊+消痔洗剂对混合痔术后并发症及抑制炎症因子表达的效果更优,值得临床推广.
Objective:Taking"Damp-heat treatment,both internal and external treatment"as the guiding ideology to study the clinical effect of Huangbai Capsule combined with Xiaozhi Lotion on postoperative complications of mixed hemorrhoids and its influence on inflammatory factors.Methods:90 cases of mixed hemorrhoid after operation were randomly divided into three groups:Huangbai Capsule Group(30 cases),Xiaozhi Lotion Group(30 cases)and Huangbai Capsule combined with Xiaozhi Lotion Group(30 cases).The changes of anal edema,pain and bleeding were recorded on the 3rd,7th and 14th day after operation in three groups,and the statistics of TCM syndrome were completed.The granulation tissue of each group was detected by HE,and the expression of NF-κB and IFN-γ was detected by ELISA.Results:All the three groups can effectively alleviate the pain,edema,bleeding symptoms and improve the symptoms of traditional Chinese medicine mixed hemorrhoids;On the 14th day after operation,Huangbai Capsule combined with Xiaozhi Lotion Group was better than Xiaozhi Lotion Group and Huangbai Capsule Group,the difference was significant(P<0.05);The healing time of Huangbai Capsule combined with Xiaozhi Lotion group was significantly shorter than that of Xiaozhi Lotion group and Huangbai Capsule Group(P<0.05);There was no significant difference in the total effective rate.On the 14th day after operation,the expression of NF-κB and IFN-γin the three groups were all decreased,but that in the Huangbai Capsule combined with Xiaozhi Lotion group was significantly lower than that in the other two groups(P<0.05).On the 14th day after operation,the fibroblasts in the Huangbai capsule combined with Xiaozhi lotion group were found to be relatively dense and orderly,and collagen fibers and more capillary were clearly seen.Conclusion:Huangbai Capsule and Xiaozhi Lotion,guided by the theory of"Damp-heat treatment,internal and external treatment",have better effect on the postoperative complications of mixed hemorrhoids and inhibiting the expression of inflammatory factors.
