瘢痕妊娠的MRI分型及临床价值OA
MRI of Cesarean Scar Pregnancy:Imaging Classification and Clinical Value
目的:探讨子宫切口瘢痕妊娠(cesarean scar pregnancy,CSP)的MRI征象及临床价值.方法:回顾性分析40例CSP患者的临床及MRI资料,包括瘢痕MRI信号特征、瘢痕厚度、孕囊类型、孕囊大小、孕囊与瘢痕之间关系等.由2名不同年资医师共同观察,评价瘢痕、孕囊及子宫的MRI特征,包括CSP的影像分型、增强强化方式及病理表现.结果:术前MRI诊断CSP的准确率为100%.40例CSP中,囊状孕囊共25例,包块型孕囊15例.MRI分型中,I型15例,II型13例,III型12例,基于MRI分型手术方式选择符合率为95%.结论:术前MRI能准确判断CSP孕囊类型、大小、瘢痕位置及子宫肌层厚度,增强MRI有助于明确CSP瘢痕附着点血供情况,术前MRI分型符合率高,对临床决策具有重要指导意义.
Objective:To investigate the MRI features and clinical value of cesarean scar pregnancy(CSP).Methods:The clinical and MRI data of 40 cases of CSP were retrospectively analyzed,including the MRI signal characteristics of scars,measurement of scar thickness,determination of gestational sac types,size of gestational sacs,and the relationship between gestational sacs and scars.Two doctors with different seniorities jointly observed and evaluated the MRI features of scars,gestational sacs,and the uterus,including the imaging classification of CSP,enhancement patterns,and pathological manifestations.Results:The accuracy rate of preoperative MRI in diagnosing CSP was 100%.Among the 40 cases of CSP,there were 25 cases of cystic gestational sacs and 15 cases of mass-type gestational sacs.In the MRI classification,there were 15 cases of type I,13 cases of type II,and 12 cases of type III.The coincidence rate of surgical method selection based on MRI classification was 95%.Conclusion:Preoperative MRI can accurately determine the type and size of the gestational sac in CSP,the location of the scar,and the thickness of the uterine muscle layer.Enhanced MRI is helpful in clarifying the blood supply at the scar attachment point.The MRI classification has a high coincidence rate and is of great significance for clinical decision-making.
