目的 分析早产儿甲状腺功能发育特点,建立不同胎龄儿甲状腺激素水平的参考区间.方法 回顾性分析 2017 年 1 月至 2022 年 12 月解放军总医院第七医学中心新生儿科收治的 2 827 例新生儿的临床资料,并依据胎龄分为超早产儿组(胎龄<28周,n=122)、极早产儿组(胎龄28~31+6 周,n=631)、早产儿组(胎龄 32~36+6 周,n=1 881)和足月儿组(胎龄>37周,n=193).采用化学发光免疫分析法检测甲状腺激素水平,比较不同胎龄、不同性别、不同日龄新生儿的促甲状腺激素(thyroid-stimulating hormone,TSH)、三碘甲状腺原氨酸(triiodothyronine,T3)、四碘甲状腺原氨酸(tetraiodothyronine,T4)、游离三碘甲状腺原氨酸(free triiodothyronine,FT3)、游离四碘甲状腺原氨酸(free tetraiodothyronine,FT4)水平,并建立相应参考区间.统计学方法采用独立样本t检验、χ2 检验、单因素方差分析、Mann-Whitney U检验和Kruskal-Wallis H检验.结果 不同胎龄组新生儿的TSH、T3、T4、FT3、FT4 水平比较,差异均有统计学意义(P值均<0.05).各胎龄组内部分甲状腺激素(极早产儿T4,早产儿T3、T4、FT3、FT4,足月儿T4)存在性别的差异(P值均<0.05).由此,根据胎龄和性别进行分组,建立针对早产儿的甲状腺激素水平参考区间.结论 早产儿甲状腺激素水平存在显著的胎龄、性别依赖性,本研究建立了基于胎龄分层的甲状腺激素水平参考范围,可为临床早期识别甲状腺功能障碍提供依据.

Objective To analyze the developmental characteristics of thyroid function in premature infants and establish reference intervals for thyroid hormone levels in infants of different gestational ages.Methods A retrospective analysis was conducted on the clinical data of 2 827 neonates admitted to the Department of Neonatology,the Seventh Medical Center of Chinese PLA General Hospital from January 2017 to December 2022.The neonates were divided into four groups based on gestational age:extremely premature infants(gestational age<28 weeks,n=122),very premature infants(gestational age 28-31+6 weeks,n=631),premature infants(gestational age 32-36+6 weeks,n=1 881),and full-term infants(gestational age>37 weeks,n=193).Chemiluminescent immunoassay was used to detect the thyroid hormone levels.The levels of thyroid-stimulating hormone(TSH),triiodothyronine(T3),tetraiodothyronine(T4),free triiodothyronine(FT3),and free tetraiodothyronine(FT4)were compared among neonates of different gestational ages,genders,postnatal ages,and corresponding reference intervals were established.Statistical analysis was performed using independent samples t-test,χ² test,One-way analysis of variance(ANOVA),Mann-Whitney U test,and Kruskal-Wallis H test.Results There were statistically significant differences in TSH,T3,T4,FT3,and FT4 levels among the different gestational age groups(all P<0.05).Within each gestational age group,gender differences were observed in some thyroid hormones:T4 in very premature infants;T3,T4,FT3,and FT4 in premature infants;and T4 in full-term infants(all P<0.05).Therefore,reference intervals for thyroid hormones in premature infants were established by grouping according to gestational age and gender.Conclusion Thyroid hormone levels in premature infants are significantly dependent on gestational age and gender.This study established reference intervals of thyroid hormone stratified by gestational age,which can provide a basis for the early clinical identification of thyroid dysfunction.
