神经重症病理生理机制复杂,在高原环境下其发病率更高、损伤更重、救治难度更大.本共识基于国内外最新循证医学证据,构建适用于高原及全国的规范化、目标导向神经重症管理框架.共识制订遵循国际证据质量评估标准,经两轮德尔菲法专家函询,形成涵盖管理体系、监测评估与核心策略三部分共32 条推荐意见.核心更新内容包括:倡导建立神经重症独立管理单元,推行基于"重症五不同"理念的精准分级诊疗;构建以脑血流、脑氧合、脑功能为核心的"三位一体"多模态脑监测体系,强调常规开展床旁经颅多普勒超声、脑氧及持续脑电图监测;在管理策略上,推动从亚低温治疗向目标体温管理转变,并明确超级重症阶段"446"目标化管理路径;强调通过多模态方法评估静态与动态脑血管自调节功能,实现个体化最佳平均动脉压管理;提升脑脊液管理目标至"类淋巴系统"功能维护层面;贯彻多学科协作全程管理模式,关注患者长期神经功能预后;降阶梯治疗依据包括脑结构恢复、脑血管自调节功能恢复、脑脊液动力学改善及生物标志物水平下降等多维指标;并将人工智能等前沿技术融入重症后管理与康复规划.本共识系统整合神经重症全流程管理,体现目标化、动态化、多模态融合的现代神经重症医学内涵,旨在适应病理生理机制深化、医工结合与人工智能赋能的新趋势,进一步推动重症医学学科发展.

Neurocritical care involves complex pathophysiological mechanisms,and its incidence is higher,injuries are more severe,and treatment is more challenging in high-altitude environments.This con-sensus,based on the latest domestic and international evidence-based medical data,establishes a standardized,goal-oriented framework for neurocritical care management applicable in high-altitude regions and nationwide.The consensus was developed following international standards for evidence quality assessment and underwent two rounds of Delphi expert consultation,resulting in 32 recommendation statements covering three parts:management systems,monitoring and assessment,and core strategies.Key updates include:advocating for the establishment of independent neurocritical care units and implementing precise tiered diagnosis and treat-ment based on the"Five Differences in Critical Care"concept;constructing a"trinity"multimodal brain moni-toring system centered on cerebral blood flow,cerebral oxygenation,and brain function,emphasizing routine bedside transcranial Doppler ultrasound,cerebral oximetry,and continuous electroencephalography monitoring;shifting management strategies from mild hypothermia therapy to targeted temperature management,and defining the"446"target management pathway for the supercritical stage;emphasizing the assessment of static and dynamic cerebrovascular autoregulation functions through multimodal methods to achieve individualized optimal mean arterial pressure management;elevating cerebrospinal fluid management goals to the level of"glymphatic system"function maintenance;implementing a multidisciplinary collaborative,whole-process management model focusing on patients'long-term neurological functional outcomes;de-escalation crite-ria include multidimensional indicators such as recovery of brain structure,restoration of cerebrovascular auto-regulation,improvement in cerebrospinal fluid dynamics,and reduction in biomarker levels;and integrating cutting-edge technologies like artificial intelligence into post-critical care management and rehabilitation plan-ning.This consensus systematically integrates the entire process of neurocritical care management,reflecting the modern connotation of goal-oriented,dynamic,and multimodal integration in neurocritical care medicine.It aims to adapt to new trends such as deepening understanding of pathophysiological mechanisms,the integra-tion of medicine and engineering,and the empowerment of artificial intelligence,thereby further advancing the discipline of critical care medicine.
