按病种分值付费对上海市某二甲综合性医院平均住院日的影响分析OA
Impact analysis of diagnosis-intervention packet payment on average length of stay in a second-level general hospital in Shanghai
目的 从按病种分值付费对某二甲医院平均住院日的影响角度出发,研究DIP支付改革下某二甲医院平均住院日的变化情况,为DIP支付改革下综合评价医疗服务和质量管理提供决策思路.方法 从某二甲医院HQMS系统中导出DIP支付前后2022年7月1日—2025年6月30日的住院病案首页信息,运用SPSS23.0对平均住院日、术前平均住院日、术后平均住院日、平均住院费用、手术占比、治疗转归、主要诊断及主要手术选择正确率等指标进行统计分析,通过t检验或卡方检验比较前后差异.结果 DIP支付后全院平均住院日下降0.6天,术前平均住院日下降0.3天,差异均有统计学意义.同时全院31个临床科室中有16个平均住院日实现下降,差异均有统计学意义,其他科室的平均住院日变动不明显.DIP支付方式改革后,CMI值增加0.080 9,DIP组数增加42组.病案首页的主要诊断和主要手术填写正确率均提高13%,卡方检验差异有统计学意义.结论 实施DIP支付后,该二甲医院转变医院管理方式,提升医院内部管理水平和医疗服务效率,提升病种收治能力,实现了平均住院日、术前及术后平均住院日的迅速下降,提升了医院治疗水平和医疗质量,降低了医疗费用.
Objective To study the changes in the average length of stay(ALOS)in a second-level general hospital un-der the Diagnosis-Intervention Packet(DIP)payment reform,and to provide decision-making insights for comprehensively evalu-ating medical services and quality management under the DIP payment reform from the perspective of its impact on ALOS.Meth-ods Hospitalization medical record front page data from July 1,2022,to June 30,2025(before and after DIP implementation)were extracted from the hospital's HQMS system.SPSS 23.0 was used to statistically analyze indicators such as ALOS,preopera-tive ALOS,postoperative ALOS,average hospitalization cost,surgical proportion,treatment outcomes,and accuracy rates of pri-mary diagnosis and primary surgery selection.T-tests or chi-square tests were employed to compare differences before and after the reform.Results After DIP implementation,the hospital's overall ALOS decreased by 0.6 days,and preoperative ALOS de-creased by 0.3 days,with statistically significant differences.Among the 31 clinical departments,16 showed a statistically signif-icant reduction in ALOS,while changes in other departments were not significant.Post-DIP reform,the Case Mix Index(CMI)increased by 0.080 9,and the number of DIP groups increased by 42.The accuracy rates of primary diagnosis and primary sur-gery documentation on the medical record front page both improved by 13%,with statistically significant differences confirmed by chi-square tests.Conclusion After implementing DIP payment,the hospital transformed its management approach,enhanced in-ternal management and medical service efficiency,improved disease treatment capacity,achieved a rapid reduction in ALOS(in-cluding preoperative and postoperative ALOS),elevated treatment levels and medical quality,and reduced medical costs.
