卵泡液HCY水平对PCOS患者颗粒细胞氧化应激和凋亡的影响OA
The impact of follicular fluid homocysteine(HCY)levels on oxidative stress and apopto-sis of granulosa cells in patients with PCOS
目的:探讨卵泡液同型半胱氨酸(HCY)对PCOS患者颗粒细胞氧化应激、凋亡及临床治疗结局的影响.方法:收集2023 年9 月至2024 年9 月于兰州大学第一医院生殖医学中心接受IVF/ICSI-ET助孕的160 例患者的颗粒细胞及卵泡液.ELISA法检测卵泡液HCY水平,根据患者是否为PCOS及卵泡液HCY水平分为 4 组:PCOS+HCY>10μmol/L(A组),PCOS+HCY≤10μmol/L(B 组),非 PCOS 患者+HCY>10μmol/L(C组),非PCOS患者+HCY≤10μmol/L(D组),比较患者的临床资料及结局.流式细胞仪检测颗粒细胞ROS水平;Real-time PCR法检测颗粒细胞SOD1、SOD2、CAT、GPX4 表达.结果:四组ROS水平(A组最高)呈递减趋势,组间差异无统计学意义(P>0.05);A组的SOD2 表达水平高于其余三组(P<0.05);CAT及GPX4 表达水平A组>B组>C组>D组(P<0.05);C组的SOD1、SOD2 表达水平最低(P<0.05);D组SOD1 表达水平高于其余三组(P<0.05).四组的优质胚胎率、胚胎着床率、临床妊娠率、活产率、妊娠高血压疾病发生率、早产率及新生儿体重比较,差异均无统计学意义(P>0.05).结论:卵泡液HCY水平升高会引起颗粒细胞抗氧化系统代偿性紊乱,但对于IVF/ICSI-ET治疗的长远期影响尚有待论证.
Objective:Discuss the impact of follicular fluid HCY on oxidative stress,apoptosis of granulosa cells,and clinical treatment outcomes in patients with PCOS.Methods:This study collected granulosa cells and follicular fluid from 160 patients who underwent IVF/ICSI-ET at our center from September 2023 to September 2024.The level of HCY in follicular fluid was measured by ELISA.According to whether the patients had PCOS and the level of HCY in follicular fluid,they were divided into four groups:PCOS+HCY>10μmol/L(Group A),PCOS+HCY≤10μmol/L(Group B),non-PCOS patients+HCY>10μmol/L(Group C),and non-PCOS patients+HCY≤10μmol/L(Group D).Flow cytometry was used to detect ROS levels in granulosa cells.Real-time PCR was performed to measure the expressions of SOD1,SOD2,CAT,and GPX4 in granulosa cells.Results:The ROS level showed a decreasing trend across the four groups,with Group A being higher,but there were no statistically significant differences among the groups(P>0.05).The expression level of SOD2 in Group A was signifi-cantly higher than that in the other three groups(P<0.05).The expression levels of CAT and GPX4 were in the order of Group A>Group B>Group C>Group D(P<0.05).Group C had the lowest expression levels of SOD1 and SOD2(P<0.05).The expression level of SOD1 in Group D was significantly higher than that in the other three groups(P<0.05).There were no inter-group differences in the rates of high-quality embryos,embryo implantation,clinical pregnancy,live birth,gestational hypertension,premature birth,and newborn weight(P>0.05).Conclu-sion:Elevated levels of HCY in follicular fluid can lead to compensatory disturbances in the an-tioxidant system of granulosa cells,but the long-term effects on IVF/ICSI-ET treatment remain to be demonstrated.
