目的:采用双样本孟德尔随机化(MR)方法探索microRNAs(miRNAs)与子宫内膜异位症(EMs)发病危险之间的潜在因果关系.方法:整合miRNA表达数量性状位点(miR-eQTL)数据和 EMs 全基因组关联研究(GWAS)数据集.采用逆方差加权法(IVW)以及加权中位数法进行MR分析,系统评估miRNAs与EMs之间的因果关系.利用MiRNet 2.0 和 miRTarBase数据库预测显著关联miRNAs的靶基因并进行功能富集分析.结果:miR-130a-3p的表达与EMs发病风险呈正相关(加权中位数:OR=1.263,95%CI:1.142~1.398,FDR校正后P=0.010;IVW:OR=1.202,95%CI:1.125~1.285,FDR校正后P=8.83×10-5),而miR-30b-5p和miR-30d-5p则表现出保护性作用,尤其与直肠阴道间隔深部浸润型EMs风险降低相关.GO功能分析显示上述 3 个miRNA的靶基因显著富集在miRNAs代谢过程和转录调控相关通路.结论:miR-130a-3p高表达是EMs潜在危险因素,而miR-30b-5p和miR-30d-5p高表达则可能具有保护作用,它们与EMs发病之间存在潜在的因果关系.这些发现为EMs的临床诊治提供了潜在的循环生物标志物.

Objective:To investigate the potential causal relationship between circulat-ing microRNAs(miRNAs)and the risk of endometriosis(EMs)using a two-sample Mendelian randomization(MR)approach.Methods:This study integrated miRNA expression quantitative trait loci(miR-eQTL)data and genome-wide association study(GWAS)summary statistics for endometriosis.Inverse-variance weighted(IVW)and weighted median methods were employed for MR analyses to systematically assess the causal relationship between miRNAs and EMs.MiRNet 2.0 and miRTarBase databases were utilized to predict target genes of significantly as-sociated miRNAs,followed by functional enrichment analysis.Results:Genetically predicted higher levels of miR-130a-3p were significantly associated with an increased overall risk of EMs(Weighted median:OR=1.263,95%CI:1.142 to 1.398,FDR-corrected P=0.010;IVW:OR=1.202,95%CI:1.125 to 1.285,FDR-corrected P=8.83×10-5).Genetically predicted high-er levels of miR-30b-5p and miR-30d-5p exhibited protective effects,particularly associated with a reduced risk of deeply infiltrating endometriosis(DIE)in the rectovaginal septum.Gene Ontology(GO)functional enrichment analysis indicated that the target genes of these three miRNAs were significantly enriched in pathways related to miRNA metabolic processes and transcriptional regulation.Conclusion:This study suggests that genetically predicted higher ex-pression of miR-130a-3p is a potential risk factor for EMs,while higher expression of miR-30b-5p and miR-30d-5p may confer protection.These miRNAs demonstrate potential causal relation-ships with EMs pathogenesis,providing candidate circulating biomarkers for the clinical diagno-sis and treatment of EMs.
