This study presents Professor Zhang Yunling's clinical experience in syndrome differentiation and the treatment of Parkinson's disease(PD)complicated by orthostatic hypotension(OH),based on the pattern differentiation of"Deficiency-Descent-Stasis".Professor Zhang posits that the pathogenesis of PD with OH involves functional impairment and deficiency of the liver,spleen and kidneys,insufficiency of qi and blood;and in prolonged illness,obstruction of the orifices by substantial solid evil such as wind,fire,phlegm,and blood stasis.He proposes the"Deficiency-Descent-Stasis"diagnostic and therapeutic philosophy,where deficiency of vital qi serves as the foundation,with tonification therapy taking precedence.Treatment involves the use of formulas such as Taishan Panshi Powder,Huqian Pills,Liuwei Dihuang Pills,Zuogui Pills,Yougui Pills to tonify essence and blood,replenish essence and fill the marrow.Descent of the middle energizer is the key,with lifting therapy as the pivot,using formulas such as Buzhong Yiqi Decoction,Juyuan Jian and Shengma Gegen Decoction to lift up the clear yang and support middle qi.Stasis-related evil is the target,with clearing methods as the key,using formulas such as Wendan Tang and Houpo Zhishi Tang to clear pathogenic factors and ensure the smooth flow of all vascular patency.
