基于数据挖掘技术探析针灸治疗血管性痴呆的选穴规律OA
Analysis of acupoint selection patterns in acupuncture treatment for vascular dementia based on data mining technology
目的:利用数据挖掘技术剖析针灸治疗血管性痴呆(VD)的选穴规律,为临床治疗提供思路参考.方法:检索中国期刊全文数据库(CNKI)2015年1月1日—2025年4月1日收录的针灸治疗VD的临床研究文献资料,筛选出符合纳入标准的文献,提取信息建立数据库,通过Excel、IBM SPSS Statistics 26.0和IBM SPSS Modeler 18.0软件对选穴规律进行描述性分析、关联规则分析及聚类分析.结果:共计纳入74篇文献,93个穴位,其中百会穴使用频次(63次)最高,处方穴位多分布于督脉和头部,使用频次≥10次的腧穴包括百会、三阴交、足三里、内关、神庭、四神聪、神门、印堂、水沟、太溪、风池、悬钟、风府、大椎;腧穴归经频次排名前5位分别是督脉、经外奇穴、足少阳胆经、足阳明胃经、手厥阴心包经;腧穴关联规则分析中关联性最强的前3位分别为百会-四神聪、百会-神庭、四神聪-内关.聚类分析得出腧穴有效聚类群3个:聚类1:百会、足三里、悬钟、太溪、风府、神庭;聚类2:四神聪、内关、风池、水沟、三阴交、神门、印堂;聚类3:大椎.结论:针灸治疗VD基于经络气血理论的选穴核心地位,重视标本兼顾、远近配穴结合,贯通脑络气血,可改善认知功能,临床效果显著.
Objective:To analyze the acupoint selection patterns in acupuncture treatment for vascular dementia(VD)using data mining technology,providing reference for clinical practice.Methods:Clinical research literature on acu-puncture treatment for VD published between January 1,2015,and April 1,2025 was retrieved from the China National Knowledge Infrastructure(CNKI)database.Literature meeting the inclusion criteria was screened,and information was ex-tracted to establish a database.Descriptive analysis,association rule analysis and cluster analysis of acupoint selection pat-terns were conducted using Excel,IBM SPSS Statistics 26.0 and IBM SPSS Modeler 18.0 software.Results:A total of 74 articles and 93 acupoints were included.Baihui(GV20)was the most frequently used acupoint(63 times).The acupoints in prescriptions were mostly distributed along the Governor Vessel and the head.Acupoints with a frequency of use≥10 times included Baihui(GV20),Sanyinjiao(SP6),Zusanli(ST36),Neiguan(PC6),Shenting(GV24),Sishencong(EX-HN1),Shenmen(HT7),Yintang(EX-HN3),Shuigou(GV26),Taixi(KI3),Fengchi(GB20),Xuanzhong(GB39),Fengfu(GV16)and Dazhui(GV14).The top five meridians in terms of acupoint frequency were the Governor Vessel,Ex-tra Nerve Points,Gallbladder Meridian of Foot-Shaoyang,Stomach Meridian of Foot-Yangming and Pericardium Meridian of Hand-Jueyin.The top three strongest association rules in acupoint association rule analysis were Baihui(GV20)-Sishen-cong(EX-HN1),Baihui(GV20)-Shenting(GV24)and Sishencong(EX-HN1)-Neiguan(PC6).Cluster analysis identi-fied three effective acupoint clusters:cluster 1:Baihui(GV20),Zusanli(ST36),Xuanzhong(GB39),Taixi(KI3),Fengfu(GV16),Shenting(GV24);cluster 2:Sishencong(EX-HN1),Neiguan(PC6),Fengchi(GB20),Shuigou(GV26),Sanyinjiao(SP6),Shenmen(HT7),Yintang(EX-HN3);cluster 3:Dazhui(GV14).Conclusion:Acupunc-ture treatment for VD is based on the core principles of meridian and Qi-blood theory,emphasizing both the root and branch,combining distal and local acupoint selection to promote the flow of Qi and blood in the brain collaterals,thereby improving cognitive function with significant clinical effects.
