目的 分析肺部超声B线数量与腹膜透析患者体质量变化、细胞外液(ECW)/总体液(TBW)的关系及评估容量负荷价值.方法 选取2021年9月至2023年8月收治的100例终末期肾病腹膜透析患者作为研究对象.比较透析前后肺部超声B线数量、ECW/TBW、体质量变化,分析肺部超声B线数量与体质量变化、超滤量、ECW/TBW的相关性,分析肺部超声和人体成分分析在容量负荷评估中的一致性.结果 腹膜透析患者透析后B线数量、ECW/TBW、体质量变化均少于或低于透析前(P<0.05).透析前B线数量与增加体质量呈正相关(P<0.05),透析后B线数量与剩余体质量呈正相关(P<0.05);透析前后B线数量的变化百分比与体质量减少量及超滤量均呈正相关(P<0.05).透析前和透析后B线数量与ECW/TBW均呈正相关(P<0.05);透析前后B线数量的变化百分比与ECW/TBW变化百分比呈正相关(P<0.05).肺部超声和人体成分分析评估腹膜透析患者容量负荷的一致性为97.00%,Kappa值为0.955(95%CI:0.831,0.999).结论 腹膜透析前后终末期肾病患者B线数量与ECW/TBW、体质量变化密切相关,且肺部超声与人体成分分析在评估患者容量负荷方面具有良好的一致性,可为腹膜透析患者治疗提供指导.

Objective To analyze the relationship between the number of lung ultrasound B-lines and body weight changes,extracellular water(ECW)/total body water(TBW)ratio in patients undergoing peritoneal dialysis,as well as its value in evaluating the volume load.Methods A total of 100 patients with end-stage renal disease(ESRD)undergoing peritoneal dialysis admitted from September 2021 to August 2023 were selected as the research subjects.The changes in the number of lung ultrasound B-lines,ECW/TBW ratio,and body weight before and after dialysis were compared,and the correlation between the number of lung ultrasound B-lines and body weight change,ultrafiltration volume,ECW/TBW,as well as the consistency of lung ultrasound and body composition analysis in volume load assessment,was analyzed.Results The number of B-lines,ECW/TBW ratioand body weight changes in peritoneal dialysis patients after dialysis were less or lower than those before dialysis(P<0.05).The number of B-lines before dialysis was positively correlated with the increase of body weight(P<0.05),and the number of B-lines after dialysis was positively correlated with the remaining body weight(P<0.05).The percentage change of B-line number before and after dialysis was positively correlated with body weight loss and ultrafiltration volume(P<0.05).The number of B lines before and after dialysis was positively correlated with ECW/TBW(P<0.05),and the percentage change in the number of B-lines before and after dialysis was positively correlated with the percentage change in ECW/TBW ratio(P<0.05).The consistency of lung ultrasound and body composition analysis in evaluating the volume load of peritoneal dialysis patients was 97.00%,and the Kappa value was 0.955(95%CI:0.831,0.999).Conclusion The number of B-lines before and after peritoneal dialysis in patients with ESRD is closely related to ECW/TBW ratio and body weight changes.Furthermore,lung ultrasound and body composition analysis have good consistency in evaluating patients'volume load,which can provide guidance for the treatment of peritoneal dialysis patients.
