目的:探讨机器人辅助微创经椎间孔腰椎间融合术(RA-MIS-TLIF)中精准置钉在腰椎退变患者术后的应用效果.方法:选取2020年12月—2024年12月首都医科大学附属北京友谊医院收治的80例腰椎退变患者,采用随机数字表法分为机器人辅助组(40例,在MIS-TLIF术中予以机器人辅助精准置钉技术)和传统徒手组(40例,在MIS-TLIF术中予以传统徒手置钉技术).比较两组患者单螺钉置入时间、手术时间、术中出血量、透视次数、置钉准确性、疼痛程度、术后首次下床活动时间、功能恢复、并发症发生情况之间的差异.结果:机器人辅助组患者的手术时间、单螺钉置入时间、术后首次下床活动时间均短于传统徒手组,术中出血量、透视次数、术后72 h ODI值均低于传统徒手组(P<0.05).机器人辅助组患者的置钉优良率高于传统徒手组,总并发症发生率低于传统徒手组(P<0.05).机器人辅助组患者术后6h、24 h、48 h、72 h的VAS评分均低于传统徒手组(P<0.05);重复测量方差分析的组间、时间及交互效应均显著(P<0.001).结论:RA-MIS-TLIF术中精准置钉技术能够显著缩短腰椎退变患者的手术时间、单螺钉置入时间及术后首次下床活动时间,提高手术置钉准确性,减轻术后疼痛,减少并发症,促进功能恢复,值得推广应用.

Objective:To evaluate the clinical efficacy of precise screw placement in robot-assisted minimally invasive transforaminal lumbar interbody fusion(RA-MIS-TLIF)for patients with lumbar degenerative disease.Methods:80 patients with lumbar degenerative disease who were treated at Beijing Friendship Hospital,Capital Medical University from December 2020 to December 2024 were enrolled.They were randomly divided into the robot-assisted group(n=40,undergoing MIS-TLIF with robot-assisted precise screw placement)and the conventional manual group(n=40,undergoing MIS-TLIF with conventional manual screw placement)using a random number table.Single screw insertion time,total operative time,intraoperative blood loss,number of fluoroscopic exposures,screw placement accuracy,pain intensity,time to first ambulation after surgery,functional recovery,and incidence of complications were compared between the two groups.Results:The robot-assisted group showed significantly shorter total operative time,shorter time for single screw insertion,and earlier time to first ambulation compared to the conventional manual group(P<0.05).Intraoperative blood loss,number of fluoroscopic exposures,and postoperative 72 h ODI scores were also significantly lower in the robot-assisted group than those in the conventional manual group(P<0.05).The excellent-good rate of screw placement was higher,and the total complication rate was lower in the robot-assisted group compared to the conventional manual group(P<0.05).VAS scores at 6 h,24 h,48 h,and 72 h after surgery were all significantly lower in the robot-assisted group than those in the conventional manual group(P<0.05).Repeated-measures ANOVA indicated significant main effects of group,time,and group-by-time interaction(P<0.001).Conclusion:Robot-assisted precise screw placement in MIS-TLIF can significantly lower operative time,single screw insertion time,and time to first ambulation in patients with lumbar degenerative disease,it can improve screw placement accuracy,alleviate postoperative pain,reduce complications,and promote functional recovery,which is worthy of clinical promotion.
