目的:探讨机器人辅助下微创治疗腰椎滑脱(LS)并腰椎间盘突出症(LDH)的效果.方法:选择2018年1月—2023年1月在本院就诊的84例LS合并LDH患者作为研究对象,采用简单随机化法将患者分为对照组(n=42)和观察组(n=42).观察组采用机器人辅助微创经椎间孔入路腰椎椎间融合术(RA-MIS-TLIF)治疗,对照组采用徒手置钉微创经椎间孔入路腰椎椎间融合术治疗.比较两组椎弓根螺钉置入准确性、手术相关指标、视觉模拟评分(VAS)和Oswestry功能障碍指数(ODI),术后并发症及植骨融合率.结果:观察组患者椎弓根螺钉置入精确度较对照组显著提升(P<0.05).观察组手术耗时显著长于对照组(P<0.05).与术前相比,两组术后各随访节点(术后即刻、术后6个月、术后12个月)的VAS评分均显著下降(P<0.05),但同期组间对比差异无统计学意义(P>0.05).两组中长期随访(术后12个月、24个月)时的ODI较术前基线值显著下降(P<0.05).两组术后12个月、24个月ODI差异无统计学意义(P>0.05).两组患者在术后并发症及植骨融合率方面的差异无统计学意义(P>0.05).结论:机器人辅助下微创治疗腰椎滑脱并腰椎间盘突出症可提高椎弓根置入准确性.

Objective:To investigate the efficacy of robot-assisted minimally invasive treatment for lumbar spondylolisthesis(LS)combined with lumbar disc herniation(LDH).Methods:84 LS patients complicated with LDH who were treated in our hospital from January 2018 to January 2023 were selected.They were randomly divided into the control group(n=42)and the observation group(n=42)using the simple randomization method.The observation group underwent robot-assisted minimally invasive transforaminal lumbar interbody fusion(RA-MIS-TLIF),while the control group underwent minimally invasive transforaminal lumar interbody fusion with manual pedicle screw placement.The accuracy of pedicle screw placement,surgical indicators,visual analogue scale(VAS)scores,Oswestry disability index(ODI),postoperative complications,and bone fusion rates were compared between the two groups.Results:The accuracy of pedicle screw placement in the observation group was significantly higher than that in the control group(P<0.05).The surgical time in the observation group was significantly longer than that in the control group(P<0.05).Compared with that before surgery,VAS scores at all follow-up timepoints(immediately after surgery,6 months after surgery,and 12 months after surgery)decreased significantly in both groups(P<0.05),but there was no statistically significant difference between the two groups at the same timepoints(P>0.05).The ODI at medium-to long-term follow-up(12 and 24 months after surgery)decreased significantly in both groups compared to that before surgery(P<0.05).There was no statistically significant difference in ODI between the two groups at 12 and 24 months after surgery(P>0.05).There was no statistically significant difference in postoperative complications or bone fusion rates between the two groups(P>0.05).Conclusion:Application of RA-MIS-TLIF in the treatment of LS complicated with LDH can improve the accuracy of pedicle screw placement.
