国产单孔腔镜手术机器人在肾部分切除术中的初步应用效果(附手术视频)OA
Preliminary application of domestic single-port robotic surgical system in partial nephrectomy(with surgical video)
目的:探讨国产术锐®蛇形臂单孔腔镜手术机器人在肾部分切除术中的初步应用效果.方法:选取2021年11月—2022年7月于南京医科大学第二附属医院泌尿外科收治的4例行术锐®蛇形臂单孔腔镜手术机器人辅助肾部分切除术患者作为研究对象,记录肿瘤直径、总手术时间、操作时间、术中出血量及热缺血时间等参数,分析患者围手术期相关资料及术后恢复情况.结果:4例患者中有3例采用经腹膜后入路,1例经腹腔入路;手术时间154.80(73.00~235.00)min,操作时间106.30(52.00~174.00)min,备机时间13.80(8.00~32.00)min;患者均未增加辅助通道,肿瘤直径为2.45(1.30~3.90)cm,其中左侧1例,右侧3例;术中出血量为65.00(10.00~150.00)mL,热缺血时间为28.80(22.00~35.00)min,术后住院时间为13.50(9.00~20.00)d.术后病理结果显示,2例为肾透明细胞癌,2例为肾血管平滑肌脂肪瘤.患者住院期间及随访过程中均未出现相关并发症.结论:国产术锐®蛇形臂单孔腔镜手术机器人应用于肾部分切除术中是安全、有效的,其蛇形臂具有灵活的操作性和精准的指向性,有助于手术顺利完成.
Objective:To evaluate the preliminary application effect of domestic"SHURUI"single-port robotic surgical system in partial nephrectomy.Methods:Four patients who underwent SHURUI® single-port endoscopic robot-assisted partial nephrectomy at the Department of Urology,the Second Affiliated Hospital of Nanjing Medical University between November 2021 and July 2022 were selected as the study subjects.Tumor diameter,total operative time,operation time,intraoperative blood loss and warm ischemia time were recorded,and perioperative data and postoperative recovery were analyzed.Results:Among the four patients,retroperitoneal approach was adopted in three cases,whereas a single case underwent transperitoneal approach.The total operative time was 154.80(73.00-235.00)min.The robotic operation time was 106.30(52.00-174.00)min,and the docking time was 13.80(8.00-32.00)min.No additional auxiliary channels were required for all patients.The tumor diameter was 2.45(1.30-3.90)cm,including one case of left side and 3 cases of right side.The intraoperative blood loss was 65.00(10.00-150.00)mL.The warm ischemia time was 28.80(22.00-35.00)min.The length of postoperative hospital stay was 13.50(9.00-20.00)d.Postoperative pathological results showed that two cases of renal clear cell carcinoma and two cases of renal angiomyolipoma.No related complications occurred in all patients after surgery or during follow-up.Conclusion:The domestically developed SHURUI® single-port endoscopic surgical robot is safe and effective for partial nephrectomy,with its serpentine arm providing flexible maneuverability and precise targeting contributing to the smooth completion of surgery.
