参芪地黄汤作为治疗糖尿病肾病的代表性方剂,其潜在作用机制可能在于:通过抗氧化效应调节铁与脂质代谢相关基因的表达;抑制缺氧诱导因子-1α、肿瘤抑制蛋白p53 等铁死亡关键调控因子;增强抗氧化酶活性,阻断脂质过氧化反应,从而减轻铁死亡介导的细胞损伤.然而,该领域的研究尚处于起步阶段,目前仍面临以下挑战:(1)机制研究有待深化.虽然已初步阐明了参芪地黄汤的部分药效机制,但其在精准调控铁死亡相关基因、信号通路以及复方各成分协同作用等方面,仍需开展更深入的分子生物学实验进行系统阐释;(2)联合应用策略亟须探索.鉴于糖尿病肾病发病机制的复杂性,单一疗法往往存在局限,未来应重点探索参芪地黄汤与西药、其他中药复方或中医外治法的联合应用策略,通过多途径协同增效,为患者提供更优化的治疗方案;(3)临床循证证据缺乏.现有研究多局限于动物实验,尚缺乏高质量的临床研究数据.未来,亟需开展大规模临床试验,以确证该方剂通过抑制铁死亡治疗糖尿病肾病的人体疗效,深入解析参芪地黄汤调控铁死亡的具体机制,将有助于挖掘其在多种疾病治疗中的潜力,为临床转化提供新的思路与靶点.

Ginseng and Astragalus and Rehmannia Decoction,as a representative formula for treating diabetic kidney disease(DKD),may exert its therapeutic effects through potential mechanisms including:regulating the expression of genes related to iron and lipid me-tabolism via its antioxidant effects;inhibiting key regulators of ferroptosis such as hypoxia-inducible factor-1α and tumor suppressor pro-tein p53,etc.;and enhancing antioxidant enzyme activity to block lipid peroxidation,thereby mitigating ferroptosis-mediated cellular damage.However,research in this field is still in its infancy and currently faces the following challenges:(1)The mechanism research requires further depth.Although preliminary research has elucidated some of the pharmacodynamic mechanisms of Ginseng and Astraga-lus and Rehmannia Decoction,more in-depth molecular biology experiments are needed to systematically explain its precise regulation of ferroptosis-related genes and signaling pathways,as well as the synergistic effects of its multiple components.(2)Combined therapy strategies urgently need exploring.Given the complexity of DKD pathogenesis,single therapeutic approaches often have limitations.Fu-ture studies should focus on exploring combined application strategies for Ginseng and Astragalus and Rehmannia Decoctio with Western medicine,other Chinese medicinal compounds,or TCM external therapies to achieve synergistic effects through multiple pathways,there-by providing patients with more optimized treatment options.(3)Clinical evidence-based is lacking.Current research is predominantly limited to animal experiments,and there is a lack of high-quality clinical research data.In the future,it is necessary to conduct large-scale clinical trials to confirm the human efficacy of this decoction in treating DKD by inhibiting ferroptosis.A thorough elucidation of the specific mechanisms by which Ginseng and Astragalus and Rehmannia Decoction regulates ferroptosis will aid in exploring its poten-tial for treating various diseases and provide new insights and targets for clinical translation.
