支气管肺泡灌洗液AOPP、AXT在儿童难治性肺炎支原体肺炎中的表达及临床意义OA
Expression and clinical significance of AOPP and AXT in bronchoalveolar lavage fluid of children with refractory mycoplasma pneumoniae pneumonia
目的 探讨支气管肺泡灌洗液(BALF)晚期氧化蛋白产物(advanced oxidation protein products,AOPP)、自分泌运动因子(autotaxin,AXT)在儿童难治性肺炎支原体肺炎(refractory mycoplasma pneumoniae pneumonia,RMPP)中的表达及临床意义.方法 前瞻性选取2022年1月至2025年2月陕西省咸阳市中心医院儿内科收治的肺炎支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)患儿118例,根据是否进展为RM-PP 分为RMPP组(42例)和普通MPP组(76例).检测BALF炎症因子[C反应蛋白(C-reactive protein,CRP)、白细胞介素-6(interleukin-6,IL-6)、降钙素原(procalcitonin,PCT)]和 AOPP、AXT 水平.比较两组患儿BALF炎症因子和AOPP、AXT水平,Spearman相关分析RMPP患儿BALF AOPP、AXT水平与炎症因子的相关性;多因素logistic回归分析儿童RMPP的影响因素,受试者工作特征(ROC)曲线分析BALF AOPP、AXT及炎症因子对儿童RMPP的预测价值.结果 与普通MPP组比较,RMPP组BALF中CRP、IL-6、PCT、AOPP、AXT 水平升高(P<0.05).高热(OR=3.314,95%CI:1.056~10.398)、胸腔积液(OR=3.262,95%CI:1.334~7.973)、AOPP(OR=1.020,95%CI:1.010~1.030)、AXT(OR=1.348,95%CI:1.182~1.537)为儿童RMPP的独立危险因素(P<0.05).BALF中CRP、IL-6、PCT、AOPP、AXT水平预测儿童RMPP的曲线下面积(area under the curve,AUC)为 0.730、0.749、0.695、0.835、0.863,BALF 中 AOPP、AXT 水平预测儿童 RM-PP 的 AUC 大于 CRP、IL-6、PCT(P<0.05);BALF 中 AOPP+AXT 预测儿童 RMPP 的 AUC 大于 AOPP、AXT单独预测(P<0.05).结论 RMPP患儿BALF AOPP、AXT水平升高,与炎症反应增强及RMPP发生有关,BALF AOPP、AXT水平联合对儿童RMPP具有良好的预测价值.
Objective To investigate the expression levels of advanced oxidation protein products(AOPP)and autocrine motility factor(AXT)in bronchoalveolar lavage fluid(BALF)of children with refractory Mycoplasma pneumoni-ae pneumonia(RMPP)and to evaluate their clinical significance.Methods A total of 118 children diagnosed with My-coplasma pneumoniae pneumonia(MPP)and admitted to Xianyang Central Hospital between January 2022 and February 2025 were prospectively enrolled.According to disease progression,patients were divided into a refractory MPP group(RMPP,n=42)and a non-refractory MPP group(n=76).Levels of inflammatory markers,including C-reactive protein(CRP),interleukin-6(IL-6),and procalcitonin(PCT),as well as AOPP and AXT in BALF were measured.Differences between groups were compared.Spearman correlation analysis was performed to assess the relationships be-tween BALF AOPP and AXT levels and inflammatory markers in children with RMPP.Multivariate logistic regression anal-ysis was used to identify independent risk factors for RMPP.Receiver operating characteristic(ROC)curve analysis was conducted to evaluate the predictive value of BALF AOPP,AXT,and inflammatory markers for RMPP.Results BALF levels of CRP,IL-6,PCT,AOPP,and AXT were significantly higher in the RMPP group than in the non-refractory MPP group(all P<0.05).Spearman correlation analysis showed that BALF AOPP and AXT levels were positively corre-lated with inflammatory markers.Multivariate logistic regression analysis identified high fever(OR=3.314,95%CI:1.056-10.398),pleural effusion(OR=3.262,95%CI:1.334-7.973),elevated AOPP(OR=1.020,95%CI:1.010-1.030),and elevated AXT(OR=1.348,95%CI:1.182-1.537)as independent risk factors for RMPP.ROC curve analysis demonstrated that the areas under the curve(AUCs)for BALF CRP,IL-6,PCT,AOPP,and AXT in predicting RMPP were 0.730,0.749,0.695,0.835,and 0.863,respectively.The predictive performance of AOPP and AXT was superior to that of CRP,IL-6,and PCT(P<0.05).Furthermore,the combination of AOPP and AXT yielded a significantly higher AUC than either marker alone(P<0.05).Conclusion BALF AOPP and AXT levels are markedly elevated in children with RMPP and are closely associated with enhanced inflammatory responses and disease progression.Combined assessment of BALF AOPP and AXT shows good predictive value for RMPP and may be useful for early identifi-cation and risk stratification in pediatric patients.
