解毒生肌膏调控Ang/Tie2/VEGF信号通路促进糖尿病溃疡血管生成OA

Jiedu Shengji Plaster Promotes Angiogenesis of Diabetic Ulcers via Regulating Ang/Tie2/VEGF Signalling Pathway

目的 基于血管内皮生长因子(VEGF)、血管生成素(Ang)及其受体酪氨酸激酶(Tie2)信号通路探讨解毒生肌膏(JDSJG)促进糖尿病溃疡修复的相关机制.方法 将48只SD大鼠随机分为正常组(12只)和造模组(36只),造模组采用高脂饲料喂养联合链脲霉素腹腔注射后手术制备背部全层皮肤缺损建立大鼠糖尿病溃疡模型后,随机分为模型组、解毒组和磺胺组进行给药,每组12只;正常组及模型组消毒后予以凡士林乳膏涂抹创面,解毒组及磺胺组分别以解毒生肌膏、磺胺嘧啶银涂抹,每日换药1次;于0、3、7、14天拍照计算创面愈合率;7、14天后取材,HE染色观察创面组织炎症、肉芽组织及血管情况;胶原染色观察胶原容积;免疫组化法检测创面Ang1、Ang2、Tie2表达及分布情况;Western Blot观察创面组织VEGF、Ang1、Ang2、Tie2蛋白水平.结果 在创面处理7、14天时,与正常组比较,模型组创面愈合率降低(P<0.01),炎症浸润明显,胶原降低(P<0.01),VEGF表达水平降低(P<0.01),Ang1、Tie2蛋白在血管周围表达减少且创面组织蛋白表达水平降低(P<0.01),而Ang2蛋白在血管周围表达增加且创面组织表达水平增高(P<0.01).与模型组比较,解毒组创面愈合率升高(P<0.01),炎症浸润减少,新生血管增多,胶原增加(P<0.01),VEGF表达水平升高(P<0.01),Ang1、Tie2蛋白在血管周围表达显著增加且创面组织蛋白表达水平增高(P<0.01),Ang2在血管表达减少且创面组织表达水平降低(P<0.01);而磺胺组仅有创面愈合率、炎症浸润、Ang1、Tie2、Ang2出现了逆转,但均弱于解毒组(P<0.01,P<0.05).结论 解毒生肌膏可在早期抗炎的同时促进血管生成,加速糖尿病溃疡愈合,且机制可能与调控Ang/Tie2/VEGF信号通路有关.

Objective To explore the mechanism by which Jiedu Shengji Plaster promotes diabetic ulcer repair based on the signalling pathways of vascular endothelial growth factor(VEGF),angiopoietin(Ang),and tyrosine kinase(Tie2).Methods Forty-eight SD rats were randomly divided into the normal control(n=12)and model groups(n=36),induced via high-fat diet combined with intraperitoneal injection of streptozotocin to create full-thickness skin defects on the back,and the rat model of diabetes ulcer was established.The model rat were randomly divided into model group,detoxification group and sulfonamide group,12 per group.After disinfection,the normal and model groups received Vaseline cream,whereas the detoxification and sulfonamide groups were treated with Jiedu Shengji Plaster and sulfonamide silver,respectively,with daily dressing changes.Wound healing rates were calculated from images obtained at days 0,3,7,and 14.After 14 days,samples were collected for histological and molecular analyses.HE staining was used for tissue inflammation,granulation tissue,and vascular conditions.Collagen volume was detected by collagen staining.Immunohistochemistry was used to detect Ang1,Ang2,and Tie2 expressions in wounds,and Western Blot for VEGF,Ang1,Ang2,and Tie2 protein levels in wound tissues.Results After 7 and 14 days of wound treatment,compared with normal group,the wound healing rate decreased(P<0.01),inflammation increased,collagen decreased(P<0.01),VEGF expression decreased(P<0.01),Ang1 and Tie2 protein expression around blood vessels and wound tissue protein expression level decreased(P<0.01),and Ang2 protein expression around blood vessels and wound tissue expression level increased in the model group(P<0.01).Compared with the model group,the detoxification group showed significantly increased wound healing rate(P<0.01),decreased inflammation,increased neovascularization,increased collagen(P<0.01),increased VEGF expression(P<0.01),increased Ang1 and Tie2 protein expression around blood vessels and wound tissue protein expression level(P<0.01),and decreased Ang2 expression in blood vessels and wound tissue expression level(P<0.01).The sulfonamide group showed a reversal in wound healing rate,inflammatory infiltration,Ang1,Tie2,and Ang2,however.However,all were weaker than the detoxification group(P<0.01,P<0.05).Conclusion Jiedu Shengji Plaster promotes angiogenesis and accelerates the healing of diabetic ulcers simultaneously with early anti-inflammatory effects,and the mechanism may be related to Ang/Tie2/VEGF signalling pathway regulation.
