四妙剂治疗痛风及高尿酸血症的研究进展OA
Research Progress of Simiao Pill(四妙丸)in Treatment of Gout and Hyperuricemia
四妙丸出自《成方便读》,具有清热利湿、消肿止痛之功效,临床广泛用于治疗痛风及高尿酸血症(hyperurice-mia,HUA),安全性良好,黄柏∶苍术∶牛膝∶薏苡仁最常用配伍比例为1∶1∶1∶2.研究证实,四妙剂可降低急性痛风性关节炎(acute gouty arthritis,AGA)中 C 反应蛋白(C-reactive protein,CRP)、红细胞沉降率(erythrocyte sedimentation rate,ESR)、糖基化终产物受体(receptor for advanced glycation end products,RAGE)、环氧合酶-2(cyclooxygenase-2,COX-2)、肿瘤坏死因子(tumor necrosis factor,TNF)-α、白细胞介素(interleukin,IL)-6、IL-1β水平,抑制核苷酸结合寡聚化结构域样受体蛋白 3(nucleotide-binding oligomerization domain-like receptor protein 3,NLRP3)/IL-1β、基质细胞衍生因子(stromal cell-derived factor,SDF)-1/趋化因子受体4(C-X-C motif chemokine receptor 4,CXCR4)信号通路,促进巨噬细胞M2极化,以减轻炎症反应、改善中医证候积分.四妙剂还可以通过降低黄嘌呤氧化酶(xanthine oxidase,XOD)、鸟嘌呤脱氨酶(guanine deaminase,GDA)、黄嘌呤脱氢酶(xanthine dehydrogenase,XDH)、腺苷脱氨酶(adenosine deami-nase,ADA)的活性,减少尿酸合成;上调有机阴离子转运体(organic anion transporter,OAT)1、OAT3、三磷酸腺苷结合盒转运蛋白 G2(adenosine triphosphate binding cassette transporter G member 2,ABCG2)的表达,下调葡萄糖转运体 9(glucose transporter 9,Glut9)、尿酸转运体 1(uric acid transporter 1,URAT 1)和 PDZ 结构域蛋白 1(PSD-95-discs large-zonula occludens 1 domain containing 1,PDZK1)的表达,调节肠道菌群促进肾脏及肠道尿酸排泄,缩短血尿酸(serum uric acid,SUA)下降时间,减轻肾脏炎性细胞浸润、肾小管间质性肾纤维化和肾小球损伤.四妙剂在治疗痛风及HUA的同时,可能纠正糖、脂代谢异常,改善胰岛素抵抗(insulin resistance,IR),减缓动脉粥样硬化进展.网络药理学显示,四妙丸治疗痛风及HUA的关键成分为小檗红碱、表小檗碱、豆甾醇、谷甾醇,核心化合物为槲皮素、山柰酚、汉黄芩素、黄芩素、β-谷甾醇,作用机制可能通过调节炎症、糖脂代谢相关信号传导通路实现.
Simiao Pill(四妙丸),from Chengfang Biandu,has the effect of clearing heat and removing dampness,reducing swelling and relieving pain,and is widely used in the treatment of gout and hyperuricemia(HUA),with good safety.The most commonly used compatibility ratio of Huangbai(Phellodendri Chinensis Cortex)∶Changzhu(Atractylodis Rhizoma)∶Niuxi(Achyranthis Bidentatae Radix)∶Yiyiren(Coicis Semen)is 1∶1∶1∶2.The research has demonstrated that Simiao For-mula can decrease the levels of C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),receptor for advanced glycation end products(RAGE),cyclooxygenase-2(COX-2),tumor necrosis factor(TNF)-α,interleukin(IL)-6 and IL-1 β in acute gouty arthritis(AGA).This is achieved through the inhibition of nucleotide-binding oligomerization domain-like receptor pro-tein 3(NLRP3)/IL-1 β,stromal cell-derived factor(SDF)-1/C-X-C motif chemokine receptor 4(CXCR4)signaling path-way,as well as romotion of macrophage M2 polarization,aiming to reduce inflammation and improve TCM syndrome scores.It can also decrease uric acid synthesis by inhibiting the activities of xanthine oxidase(XOD),guanine deaminase(GDA),xanthine de-hydrogenase(XDH)and adenosine deaminase(ADA).It up-regulates the expressions of organic anion transporter(OAT)1,OAT3 and adenosine triphosphate binding cassette transporter G member 2(ABCG2),while down-regulating the expression of glucose transporter 9(GLUT9),uric acid transporter 1(URAT1)and PSD-95-discs large-zonula occludens 1 domain contai-ning 1(PDZK1).Additionally,it regulates intestinal flora to enhance renal and intestinal excretion of uric acid.This intervention can reduce the time for decreasing the serum uric acid(SUA)levels,alleviate renal inflammatory cell infiltration,as well as tubu-lointerstitial renal fibrosis and glomerular injury.While managing gout and hyperuricemia,Simiao Pill can potentially normalize abnormal glucose and lipid metabolism,enhance insulin resistance(IR)and decelerate the progression of atherosclerosis.Net-work pharmacology has revealed that the key components of Simiao Decoction in treating gout and HUA include berberine,epicberberine,stigmasterol and sitosterol;while the core compounds consist of quercetin,kaempferol,baicalein,baicalein and β-sitosterol.The mechanism of action may involve regulating signal transduction pathways associated with inflammation as well as glucose and lipid metabolism.
